About this file
This is a Criminal History Screening Practices Certification form required by Broward County, Florida for the FEMA Compliant Pipe and Valve Maintenance & Installation Services contract. The certification applies to all vendors seeking contracts valued at $100,000 or more with Broward County. Vendors must certify that they have implemented or will implement upon contract award policies, practices, and procedures that prohibit inquiry into an applicant's criminal history until the applicant has been selected as a finalist and interviewed for the position. This requirement applies only to positions located within the United States that will foreseeably perform work under the Broward County contract.
Vendors must complete and return the certification with their submittal or within three business days of the County's request, with failure to comply resulting in the vendor being deemed nonresponsive. Non-compliance with the criminal history screening requirements during the contract term constitutes material breach, which may result in contract termination and potential debarment or suspension proceedings. Vendors may claim exemption from these requirements only if required by applicable federal, state, or local law to conduct criminal background checks at a time or manner otherwise prohibited, or if the vendor is a governmental agency. The form was last updated on September 9, 2024.
