General Construction IDIQ Past Performance Questionnaire

Thank you in advance for filling out this questionnaire for the U.S. Department of Commerce, National Institute of Standards and Technology (NIST). We understand that your time is valuable, and we appreciate your input. Your rating is essential in successfully evaluating your contractors past performance. The following information, once submitted, will be treated as confidential and will not be released.

General Instructions Please complete the questionnaire utilizing the following guidance:

1. This form should be completed by the Government Contracting Officer or the Corporate Official who oversaw the contractor’s performance.

2. Handwriting responses are sufficient.

3. Please provide an adjectival rating for each question (adjectival ratings are defined below).

4. Please provide comments to substantiate the assigned rating.

5. Please sign, date, and email completed questionnaires to Amos.Jackson@nist.gov with a curtesy copy to the requesting firm using the email subject line:

“PPQ – 1333ND22QNB190089”

NOTE: The requesting firm (Offeror) must submit completed questionnaires to the Government for review in accordance with the proposal instructions outlined in the RFQ/RFP.

Adjectival Ratings The following adjectival ratings and definitions shall be used to evaluate the contractor’s performance:

RATING
DEFINITION
Exceptional (E)
Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element being assessed was accomplished with few, if any, minor problems for which corrective action taken by the Contractor was highly effective.
Very Good (VG)
Performance meets contractual requirements and exceeds some to the Government/Owner’s benefit. The contractual performance of the element being assess was accomplished with some minor problems for which corrective action taken by the Contractor were effective.
Satisfactory (S)
Performance meets minimum contractual requirements. The contractual performance of the element being assessed contains some minor problems for which corrective action taken by the Contractor were satisfactory.
Marginal (M)
Performance does not meet some contractual requirements. The contractual performance of the element being assessed reflects a serious problem for which the Contractor has not yet identified corrective actions. The Contractor’s proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory (U)
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element being assessed contains serious problems for which the Contractor’s corrective actions appear, or were, ineffective.
Not Applicable (NA)
No information or does not apply to your contract.
