About this file
This is a Drug-Free Workplace Certification form (VA Form 40-0895-8) issued by the Department of Veterans Affairs for a State Cemetery Grant related to a Veterans Cemetery Columbarium Expansion project (Project #26361490) in Utah. The certification document outlines requirements that grantees must meet to maintain compliance with the Drug-Free Workplace Act of 1988. The form requires grantees to certify that they will maintain a drug-free workplace by publishing statements notifying employees of prohibited controlled substance activities, establishing drug-free awareness programs, ensuring all grant-performing employees receive copies of workplace policies, requiring employees to report criminal drug convictions within five days, and notifying the VA within ten days of receiving such notice. Grantees must take appropriate personnel action or require participation in approved rehabilitation programs within 30 days of receiving notice of employee convictions, and must maintain good faith efforts to continue drug-free workplace implementation.
The certification is required prior to grant award and represents a material representation of fact. False certification or violation of the certification terms may result in suspension of payments, suspension or termination of grants, or government-wide suspension or debarment. The form requires completion of fields identifying the project site address, organization name, Federal Application Identifier number, authorized state or eligible county government representative name and title, and signature with date. The public reporting burden for completing this certification is estimated at 15 minutes, with OMB Number 2900-0559 and an expiration date of October 31, 2026. The underlying project involves expansion of an existing veterans cemetery with construction of six columbarium niche walls, associated concrete walkways, and landscape and irrigation improvements, with a construction cost estimate of $3,000,000.
