About this file

This is a Conflict of Interest Questionnaire (Form CIQ) issued by the Texas Ethics Commission for vendors conducting business with local governmental entities in Texas. The form is designed to ensure compliance with Chapter 176 of the Texas Local Government Code and requires vendors to disclose potential conflicts of interest involving local government officers or their family members. Vendors must file the completed questionnaire with the records administrator of the local governmental entity no later than the seventh business day after the vendor becomes aware of facts requiring disclosure or after beginning discussions or negotiations to enter into a contract with the local governmental entity.

The questionnaire requires vendors to disclose employment or business relationships with local government officers or their family members that result in taxable income exceeding $2,500 during the preceding 12-month period, gifts with an aggregate value exceeding $100 provided to officers or their family members within the preceding 12-month period, and any family relationships with local government officers. Vendors must also disclose business relationships maintained with corporations or entities in which a local government officer serves as an officer, director, or holds an ownership interest of one percent or more. Failure to file or knowingly violating the disclosure requirements constitutes a misdemeanor offense. The form provides definitions and references to relevant sections of the Texas Local Government Code to guide vendor compliance.
