REPORT

INSPECTION SITE: 820 North Main Street

Cedar City Utah 84721

SCOPE OF WORK: An asbestos survey inspection was performed for the purpose of determining the presence of asbestos containing materials in a portion of a Commercial Building that will be demolition. A total of 6 samples were taken and sent to the laboratory for analysis.

RESULTS: The inspection site, built between 1940-1950 is a Commercial Building that will be demolish. Samples were collected in the big room, hallway, warehouse and boiler room. Samples consisted of the thermal systems insulation (TSI) pipe wrap. The overall condition of the samples were in fair condition. All 6 samples were sent to the laboratory for analysis. The laboratory report states that there was no asbestos detected.

Federal and state OSHA regulations for any asbestos containing materials require that workers follow

OSHA work practices and procedures, including respirator use and training.

CAUTION: One percent or more of asbestos is considered to be a hazardous substance and must be removed by a Utah

Division of Air quality-approved contractor, following US EPA approved procedures.

According to the US EPA:

1. Inhalation is the primary route of exposure for asbestos.

2. There is no known safe amount of asbestos exposure.

The purpose of this report is to present the results of a pre-demolition asbestos survey, universal hazardous waste assessment, and oil-water separator outfall locate performed on November 14 and 15, 2018, at the above-referenced facility in Cedar City, Utah. This survey was conducted in accordance with Terracon Proposal Number P61187469, dated November 1, 2018. We understand that this survey was requested due to the future redevelopment of the site.

Asbestos-containing materials and universal waste/hazardous materials were identified during the assessment. Please refer to the attached report for details. PRE-DEMOLITION ASBESTOS SURVEY,

UNIVERSAL WASTE ASSESSMENT, AND LOCATION OF OIL-WATER

SEPARATOR OUTFALL

1.0 INTRODUCTION

Terracon Consultants, Inc. (Terracon) conducted an asbestos survey, universal waste assessment, and identified the location of the oil-water separator outfall at the U.S. Forest Service Compound, located at 820 North Main Street in Cedar City, Utah. The survey was conducted on November 14 and 15, 2018, by a State of Utah-certified Asbestos Inspector in accordance with Terracon Proposal Number P61187469, dated November 1, 2018. All observed materials were surveyed and homogeneous areas of suspect asbestos-containing materials (ACM) were visually identified and documented. Although reasonable effort was made to survey accessible suspect materials, additional suspect but un-sampled materials could be present in walls, in voids or in other concealed areas.

Samples of suspect ACM were collected to determine asbestos content. Sampling was performed in compliance with protocols outlined in United States Environmental Protection Agency (EPA) regulation 40 CFR 763, Asbestos Hazard Emergency Response Act (AHERA), and in accordance with the provisions of the Federal National Emission Standard for Hazardous Air Pollutants (NESHAP; 40 CFR 61, Subpart M). Upon collection, the samples were delivered to EMLab P&K in Phoenix, Arizona (National Voluntary Laboratory Accreditation Program (NVLAP) #500031-0), for analysis by polarized light microscopy (PLM), using EPA Method 600/R-93/116.

1.1 Project Objective

The objective of this project was to identify the presence or absence of asbestos, universal waste, and other hazardous materials present on the interior and exterior spaces of the building, and to locate the outfall pipe of the oil-water separator located in the maintenance building.

EPA regulation 40 CFR 61, Subpart M, NESHAP prohibits the release of asbestos fibers to the atmosphere during renovation or demolition activities. The asbestos NESHAP requires that regulated ACM be identified, classified and quantified prior to planned disturbances or demolition activities.

In compliance with federal guidelines, the following materials were visually identified and quantified:

n Mercury-vapor-containing fluorescent light bulbs n PCB-containing ballasts or transformers n Refrigeration units containing chlorofluorocarbons n Containers of liquid or hazardous waste n Suspect CFC-containing HVAC units n Air conditioning units n Compact fluorescent light bulbs (CFL)

The above-noted materials, if present, should be removed and disposed of or recycled should this building be demolished.

2.0 BUILDING / AREA DESCRIPTION

The U.S. Forest Service Compound consists of several vacant buildings. These buildings are a flammable materials storage shed, a sign shop, two shed/storage buildings, and the main maintenance/office building.

3.0 FIELD ACTIVITIES

The asbestos survey and universal waste assessment for the property at 820 North Main Street was conducted on November 14 and 15, 2018, by Mr. Michael Dunbar. Mr. Dunbar is a Utah-certified Asbestos Inspector. Copies of Mr. Dunbar’s most recent Asbestos Inspector refresher course certificate and license are attached as Appendix E.

3.1 Visual Assessment

Terracon began the asbestos sampling activities with a visual assessment and the identification and inventory of readily visible and accessible homogeneous areas of suspect ACM. A homogeneous area consists of materials that appear similar throughout in terms of color, texture and date of application. The interior assessment was conducted throughout visually accessible areas of the site. Materials identified as concrete, glass, wood, masonry, metal, plastic, or rubber were not considered suspect ACM.

3.2 Physical Assessment

A physical assessment of each homogeneous area of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the EPA as a material that can be crumbled, pulverized or reduced to powder by hand pressure when dry. Friability was assessed by physically touching suspect materials.

3.3 Sample Collection

Based on results of the visual observation, bulk samples of suspect ACM were collected from the accessible interior and exterior building spaces. Bulk samples of suspect ACM were collected in accordance with AHERA sampling protocols, using wet methods as applicable to reduce the potential for fiber release. Samples were placed in sealable containers and labeled with unique sample numbers, using an indelible marker. Terracon collected 94 samples from 32 homogeneous areas of suspect ACM. Sampling location drawings are provided in Appendix A. Descriptions of the suspect materials, sample locations, and laboratory results are summarized in Table 1 located in Appendix B. Although reasonable effort was made to survey accessible suspect materials, additional suspect but un-sampled materials could be in walls, in voids, or in other concealed areas.

3.4 Sample Analysis

Samples of suspect ACM were delivered under standard chain-of-custody protocol to EMLab P&K (NVLAP #500031-0) in Phoenix, Arizona, for analysis by PLM, using EPA Method 600/R-93/116.

The percentage of asbestos, where applicable, was determined by microscopic visual estimation.

Copies of the analytical reports and chain-of-custody forms are provided in Appendix C.

4.0 REGULATORY OVERVIEW

4.1 Asbestos

The Utah Division of Air Quality (UDAQ) enforces the Asbestos NESHAP adopted by reference in Section 214-1, Utah Air Conservation Rules. The owner or operator must provide UDAQ with written notification at least 10 working days prior to the commencement of asbestos abatement activities that will disturb regulated asbestos-containing material (RACM) in amounts greater than or equal to 3 square feet or 3 linear feet.

The asbestos NESHAP (40 CFR Part 61, Subpart M) regulates asbestos fiber emissions and asbestos waste disposal practices. The asbestos NESHAP regulation also requires the identification and classification of existing ACM according to friability prior to demolition or renovation activity. Friable ACM is a material containing more than 1% asbestos that, when dry, can be crumbled, pulverized or reduced to powder by hand pressure. All friable ACM is considered

RACM.

The asbestos NESHAP regulation classifies ACM as either RACM, Category I non-friable ACM or Category II non-friable ACM. RACM includes all friable ACM, along with Category I and Category II non-friable ACM that has become friable, will be or has been subjected to sanding, grinding, cutting or abrading, or ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder during renovation or demolition activity. Category I non-friable ACM are exclusively asbestos-containing packings, gaskets, resilient floor coverings, resilient floor covering

Category II non-friable ACM are all other non-friable materials other than Category I non-friable ACM that contain more than 1% asbestos. Category II non-friable ACM generally includes but is not limited to cementitious material such as cement pipes, cement siding, cement panels, glazing, mortar and grouts.

The United States Occupational Safety and Health Administration (OSHA) Asbestos Standard for Construction (29 CFR 1926.1101) regulates workplace exposure to asbestos. The OSHA standard requires that employee exposure to airborne asbestos must not exceed 0.1 fibers per cubic centimeter of air (0.1 f/cc) as an eight-hour time weighted average (TWA) and not exceed 1.0 fibers per cubic centimeter of air (1.0 f/cc) over a 30-minute time period known as an excursion limit (EL).

The TWA and EL are known as OSHA’s asbestos permissible exposure limits (PELs). The OSHA standard classifies construction and maintenance activities that could disturb ACM, and specifies work practices and precautions that employers must follow when engaging in each class of regulated work. The standard also specifies requirements for disturbing and handling materials with asbestos concentrations less than or equal to one percent.

4.2 Hazardous Materials

In compliance with federal guidelines the following materials were identified and quantified should any demolition be conducted as part of this project:

n Mercury-vapor-containing fluorescent light bulbs n PCB-containing ballasts or transformers n Refrigeration units containing chlorofluorocarbons n Containers of liquid or hazardous waste n Suspect CFC-containing HVAC units n Air conditioning units n Compact fluorescent light bulbs (CFL)

The above materials were visually identified, but no samples were collected for analysis.

5.0 FINDINGS AND RECOMMENDATIONS

5.1 Asbestos

Terracon collected 94 bulk samples from the site; based on the laboratory analytical reporting, three of the materials was identified as asbestos-containing material. The floor tile located in the maintenance building contained 2% Chrysotile, the joint compound of the gypsum board ceilings located in the maintenance building contained 2% Chrysotile, and the black roof tar located around roof penetrations of the maintenance building contained 15% Chrysotile. The floor tile located in the west shed by the south gate contained less than 1% chrysotile.

Resilient floor coverings and roofing materials are Category I Non-friable materials and do not have to be removed prior to demolition in accordance with the Utah Department of Air Quality and the EPA NESHAP rules and regulations. However, if these materials are to remain in the buildings during demolition, all demolition contractor personnel must be trained according to the requirements of the OSHA Asbestos standard for construction (29 CFR 1926.1101), the demolition would have to be performed in accordance with OSHA Class II removal requirements, and all of the demolition debris would have to be disposed of in sealed, leak-tight containers.

Therefore, Terracon recommends that these materials be removed by a certified abatement contractor prior to demolition.

Composite results of the wall system (drywall and joint compound) indicate that the system contains less than 1% asbestos and, therefore, is not regulated by EPA or Utah DAQ; however, the material must be removed following OSHA regulations. Terracon recommends that the wall system be removed and disposed of as an asbestos-containing material by certified abatement contractor prior to demolition.

The interior of the boiler in the maintenance building was not surveyed; therefore, suspect materials inside the boiler should be assumed to be asbestos-containing until tested and shown to be otherwise.

Material descriptions, locations, and sample results are summarized in Appendix B. Laboratory analytical reports are included in Appendix C.

5.2 Hazardous Materials

The following universal and hazardous waste materials were identified during this assessment:

n Approximately 364 4-foot fluorescent light tubes n Approximately 49 compact fluorescent lamps n Approximately 2 gallons of paint and 7 spray paint cans n Approximately 125 gallons of various chemicals n Approximately 2 suspect CFC-containing refrigeration units n Approximately 1 battery n Approximately 3 tires n Approximately 2 oil storage tanks n Approximately 2 hydraulic lifts n There is an unknown quantity of coal in the boiler room.

The noted materials above should be disposed of or recycled in accordance with all local, state, and federal regulations.

5.3 Oil-Water Separator Outfall Pipe

On November 15, 2018, Mr. Rooter Plumbing of St. George located the outfall pipe of the oil-water separator located in the maintenance building. The outfall pipe was traced using a camera system with a transmitter that locates the camera inside the pipe. The oil-water separator is located in the south west corner of the maintenance building. The outfall pipe runs south out of the building to the nearby manhole and turns west towards Main Street. Blockages in the pipe prevented the camera from traveling further west. It is believed that the outfall pipe either turns into the sewer line near the manhole cover or continues parallel to the sewer line and runs into Main Street. If more information is required on the location of the outfall pipe, the blockages will need to be cleared, such as by hydro-blasting.

5.4 Inaccessible Areas

The outfall pipe from the oil-water separator had a blockage near the manhole cover south of the maintenance building, which the camera system could not get past.

The interior of the boiler located in the maintenance building was not inspected and may contain asbestos.

6.0 GENERAL COMMENTS

This survey was conducted in a manner consistent with the level of care and skill ordinarily exercised by members of the profession currently practicing under similar conditions in the same locale. The results, findings, conclusions and recommendations expressed in this report are based on conditions observed during our survey of the building. The information contained in this report is relevant to the date on which this survey was performed and should not be relied upon to represent conditions at a later date. This report has been prepared on behalf of and exclusively for use by the Case, Lowe, & Hart, Inc. for specific application to their project as discussed. This report is not a bidding document. Contractors or consultants reviewing this report must draw their own conclusions regarding further investigation or remediation deemed necessary. Terracon does not warrant the work of regulatory agencies, laboratories or other third parties supplying information which may have been used in the preparation of this report. No warranty, express or implied, is made.

All samples were received in acceptable condition unless noted in the Report Comments portion in the body of the report. The results relate only to the items tested. The results include an inherent uncertainty of measurement associated with estimating percentages by polarized light microscopy. Measurement uncertainty data for sample results with >1% asbestos concentration can be provided when requested.
