About this file
This document is a Special Contract Requirements section for a federal construction contract, outlining safety, traffic control, personnel qualifications, and environmental compliance obligations for the contractor.
The contractor must establish a comprehensive Accident Prevention Plan before on-site work begins, subject to CO approval, covering a responsible supervisor, weekly and monthly safety meetings, first aid procedures, work phase hazards and mitigation methods, employee training, safety inspection reports, and employee qualification documentation. The plan cannot proceed to progress payments until approved by the Contracting Officer. Reportable accidents—defined as death, occupational disease, traumatic injury, property damage exceeding $100, and fires—must be reported within 48 hours and documented on form DI-134 within 7 days. The contractor must comply with OSHA/CalOSHA standards, provide adequate first aid facilities and personnel protective equipment (including cut-resistant chain saw chaps meeting UL or Forest Service specifications), maintain daily equipment inspections, conduct weekly tailgate safety meetings and monthly supervisory meetings with CO notification, and ensure all personnel wear appropriate hard hat area protective equipment. A Traffic Control Plan must be submitted at least 15 days before construction starts, accommodating MUTCD and FHWA standards, with full closure of M6 between MP 19.1 and MP 24.7 and appropriate signage and Type III barricades at designated intersections.
Key personnel must include a Project Manager, Site Superintendent (minimum five years remote project experience), Project Quality Control Manager (minimum three years CQC experience), Health and Safety Manager, and Erosion Control/SWPP Manager (minimum three years experience), with substitutions restricted during the first 90 days except for illness, death, or termination. Additional requirements include archaeological and historic site protection procedures, spill prevention and countermeasures planning if oil storage exceeds 1,320 gallons, water draw restrictions using 3/8-inch maximum intake filters from the Eel River or Lake Pillsbury, on-site contractor superintendence with English-language proficiency, and assumption of liability for contractor-caused losses except where Government negligence applies.
