About this file

This is a solicitation evaluation criteria document issued by the State of Nebraska Department of Health and Human Services for solicitation number 125999 O3, which seeks a qualified bidder to provide a Nebraska Olmstead Plan Evaluation. The solicitation opening date is July 27, 2026, at 2:00 PM. Bidders must submit responses that include a corporate overview, technical response, completed cost sheet, completed solicitation sections II through IV, and a manually signed or DocuSign-executed contractual agreement form to be considered responsive.

Evaluation of responsive proposals will be conducted on a 100-point scale without vendor demonstrations or up to 120 points if vendor demonstrations are required. The scoring breakdown allocates 25 points for corporate overview, 55 points for technical response, and 20 points for cost. Cost points are awarded using a formula that grants maximum points to the lowest realistic cost submitted, with other bids scored proportionally using the ratio of lowest cost to submitted cost multiplied by the maximum 20 possible cost points.

Lowest Cost Submitted ÷ Cost Submitted x Maximum Possible Cost Points = Cost Points to Award (see samples below)

Formula Sample Sample Sample
