ATTACHMENT 1

PERFORMANCE WORK STATEMENT

Application of Calcium Chloride

SECTION 1

GENERAL

1.1. SCOPE OF WORK. Calcium Chloride as defined in this Performance Work Statement (PWS) except for those Government Furnished items is specified in Section 3. These services are to be performed at (specific location) or (location shown on the attached map(s). The contractor shall perform in accordance with the standards defined in this contract. The estimated quantities of work are listed on page of SF 1449. The contractor shall provide all personnel, equipment, labor, supervision, transportation, supplies and materials (including safety), and all other incidentals necessary to apply Calcium Chloride on designated roads on the Savannah River Site (SRS) secondary road system. All work shall be performed in accordance with contract clauses and specifications contained or referenced herein. Calcium Chloride shall be applied on the secondary roads of the SRS. A map will be provided for awarded offeror. Access to all work shall be readily available except for road or security restrictions, which may entail using an alternate route to the road being treated. The Contractor is to coordinate all movement of equipment with the Contracting Officer's Representative (COR).

1.1.2 - Equipment Requirements

1.1.2.1 All equipment proposed for use in the performance of the work shall be of the size, type and in satisfactory operating condition to fully perform each task. Any movement or transportation of equipment to and from the work areas for repair, replacement, or for Contractor's convenience shall be at the Contractor's expense.

1.1.2.2 The Contractor shall furnish operators, helpers, all fuel, oil, grease, repairs, and pay any other expenses incidental to the operation of the equipment.

1.1.3 - Public Safety and Traffic Control

1.1.3.1 The Contractor shall exercise caution and care while pursuing the work in order to prevent unnecessary conflict with, or potential hazards to, road users.

1.1.3.2 The Contractor shall perform all on site work in accordance with all Federal, State and Local laws governing the transport of equipment and type of work being performed.

1.1.3.3 The Contractor shall post warning signs at each end of the section being worked. Signs are to be of the size, quantity, and color as required in the current edition of the "Manual on Uniform Traffic Control Devices", (MUTCD). A copy of MUTCD is located at the Forest Service/ Savannah River Engineering and Environmental Services Building and may be made available to the Contractor for an in office reference upon request. These signs shall be moved as needed to properly delineate and identify the section of road that the contractor is working on. At no time shall these signs be more than two miles from operating equipment. Signs shall be in place only when operations are being performed. All slow moving equipment is to have a properly attached reflectorized "slow moving" vehicle emblem.

1.1.3.4 All work requiring closure of a road must be scheduled and approved by the COR prior to closure. All work is to be scheduled so that at the end of a working day the road is passable for the type of traffic normally using the road.

1.2. PERSONNEL.

1.2.1 CONTRACTOR REPRESENTATIVE. The contractor shall designate a representative during performance of the contract and until the work is completed and accepted, the Contractor shall directly supervise the work or assign a competent superintendent to be at the work site at all times. The superintendent shall be designated in writing with a copy of such designation provided to the Contracting Officer prior to the contract start date. The superintendent must have authority to act for the Contractor, and be acceptable to the Contracting Officer.

1.2.1.1 Authority. The contractor representative shall have authority to act for the contractor on all contract matters relating to daily operation of this contract.

1.2.1.2. Response Time.

1.2.1.2.1 The contractor representative shall be available during normal duty hours, at the site of work, with Government personnel designated by the contracting officer to discuss and resolve performance issues.

1.2.1.2.2 Task orders may be placed throughout the base and option periods noted in the schedule of items. Task orders issued by the Government will detail the locations to be serviced, the services to be provided, and the order in which performance is required. Actual quantities of services ordered will depend upon existing conditions at the time of the Government survey. The Contractor will be required to have the equipment necessary to complete the task orders for each delivery order period.

1.2.1.3. Language. The contractor representative must be able to read, write, speak, and understand English.

1.2.2.EMPLOYEES:

1.2.2.1. Certifications. The contractor shall ensure that employees have the following current and valid professional certifications before starting work. Proper drivers license, based upon equipment being operated (e.g. Commercial Drivers License).

1.2.3. SECURITY OR ACCESS REQUIREMENT.

1.2.3.1. Pass and Identification. The contractor shall obtain the following pass and identification items: U.S. Department of Energy Savannah River Photo ID and site clearance badge. This will require General Employee Training (GET), which involves up to one day of training provided by Savannah River Site.

1.3 QUALITY CONTROL.

1.3.1. QUALITY CONTROL PLAN. The contractor shall establish and maintain a Quality Control Program to ensure the requirements of the contract are achieved. One copy of the contractor's Quality Control Plan shall be provided to the contracting officer prior to the start of contract work. The Contractor is responsible for supervision and inspection procedures necessary to insure that all contract performance specifications are met, and that all equipment used is maintained in good and safe working condition. The contractor shall monitor field operations to insure that Calcium Chloride application procedures are being performed on the designated roads as specified on the task order. Contractor shall provide the COR with the application standards that are used for Calcium Chloride.

1.3.2. PLAN CONTENTS.

1.3.2.1. Inspection System. An inspection system must cover all the services listed on the Performance Requirements Summary (PRS). The contractor will need to indicate what the application standards will be used for the calcium chloride. (manaufacturer’s standard or government suggested rate of application). It must specify the areas to be inspected on both a scheduled or unscheduled basis, how often inspections will be accomplished, and the title of the individual(s) who will perform the inspection. The inspection system must provide for a written record of inspections and results.

1.3.2.2. Methods. The methods for identifying and preventing defectives in the quality of service performed before the level of performance becomes unacceptable.

1.3.2.3. Records. On-site records of all inspections conducted by the contractor and necessary corrective action taken. The Contractor shall maintain records of accomplishments with metered delivery tickets. Tickets for each delivery and application shall be provided to the government representative for signature. The metered delivery tickets shall be submitted with the invoice for verification and payment. This documentation shall be made available to the contracting officer during the term of the contract.

1.3.3. GOVERNMENT REVIEW. The Government will review the Contractor Quality Control Plan to determine if it is adequate. If the plan is found to be adequate, the Government will authorize the plan. If the plan is not adequate as submitted, the deficiencies will be identified to the contractor in writing. The contractor shall correct all identified deficiencies and submit a corrected Contractor Quality Control Plan within ten working days or contract start date, whichever is sooner. Contractor performed work which fails to conform to standards when the contracting officer performs quality assurance work will serve as an indicator that the contractor QC system is not adequate.

1.4. QUALITY ASSURANCE.

1.4.1. CONTRACTING OFFICER'S REPRESENTATIVE (COR). COR(s) will be appointed to monitor contractor performance and have the right to inspect and accept or reject the services as required by the contract. The name and telephone number of each COR under the contract will be furnished to the contractor in writing by the contracting officer.

1.4.2. GOVERNMENT MONITORING.

1.4.2.1 The Government will monitor the contractor's performance under this contract using the method(s) of surveillance specified in the PRS. All monitoring observations will be recorded by the government. When an observation indicates defective performance, the COR will request the contractor's representative to confirm defective work and perform corrective work.

1.4.2.2 The following government officials will participate in assessing the quality of the contractor’s performance. Their roles and responsibilities are described as follows: The Contracting Officer’s Representative (COR) and is responsible for monitoring, assessing, recording, and reporting on the technical performance of the contractor on the schedule of inspections submitted by the contractor and approved by the government.

1.4.3. PERFORMANCE EVALUATION MEETINGS. The contract manager may be required to meet weekly with the COR or others during the first month of the contract. Meetings shall be as often as necessary thereafter as determined by the contracting officer. However, if the contractor requests, a meeting will be held whenever a Contract Discrepancy Report is issued. The meetings shall focus on contractor performance and actual or potential problems. A mutual good faith effort shall be made by all parties to resolve all issues. The written minutes of these meetings shall be signed by the parties to the discussion. Should the contractor not concur with the minutes, the contractor shall so state any areas of nonconcurrence in writing to the contracting officer within seven work days of receipt of the minutes.

1.5. PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all Government property provided for contractor use in accordance with the Government Furnished Property clause, 52.245-4.

1.5.1. KEY CONTROL. The contractor shall establish and implement methods of ensuring that all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. No keys issued the contractor by the Government shall be duplicated. The contractor shall develop procedures covering key control that shall be included in the quality control plan.

1.6. HOURS OR OPERATION.

1.6.1. NORMAL HOURS. The contractor shall maintain the following customer service hours: Monday – Friday 0600 – 1800.

1.6.1.1. Recognized Holidays. The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day.

1.8. DOCUMENTATION AND RECORDS. All documentation, records and schedules as described in this PWS that are the responsibility of the contractor, are the property of the Government and shall be turned over to the Government upon completion or termination of the contract. All records are subject to the Freedom of Information and/or Privacy Acts and any requests for release of any records shall be handled accordingly. See section 1.3.2.3 regarding metered delivery tickets.

SECTION 2

DEFINITIONS

2.1. STANDARD DEFINITIONS.

2.1.1. ACCEPTABLE QUALITY LEVEL (AQL). The maximum percent defective (or the maximum number of defects per hundred units) that for purposes of random sampling, may occur before the Government will effect the price computation system in accordance with the Performance Requirements Summary and the Inspection of Services Clause. When the method of surveillance is other than random sampling, the AQL is defined as the number of defects or maximum percentage of defects in the lot that may occur before the Government will affect the price computation system in accordance with the Performance Requirements Summary and the Inspection of Services Clause. An AQL does not allow a contractor to knowingly offer defective service, but limits reduced payment to circumstances in which defective performance results in a measurable reduction in the value of services rendered.

2.1.2. DEFECTIVE SERVICE. A unit of service, which contains one or more defects, or nonconformance with specified requirements.

2.1.4. PERFORMANCE REQUIREMENTS SUMMARY (PRS). Identifies the key service outputs of the contract that will be evaluated by the Government to assure contract performance standards are met by the contractor.

2.1.5. QUALITY ASSURANCE. Those actions taken by the Government to assure services meet the requirements of the Performance Work Statement (PWS).

2.1.6. CONTRACTING OFFICER'S REPRESENTATIVE (COR). A Government person designated by the contracting officer with authority to act within the scope the delegated authority.

2.1.7. QUALITY ASSURANCE SURVEILLANCE PLAN (QASP). An organized written document used for quality assurance surveillance. The document contains specific methods to perform surveillance of the contractor.

2.1.8. QUALITY CONTROL. These are actions taken by a contractor to control the performance of services so that services meet the requirements of the PWS.

2.1.9. RANDOM SAMPLING. A sampling method in which each service output in a lot has an equal chance of being selected.

2.1.10. SAMPLE. A sample consists of one or more service outputs drawn from a lot. The number of outputs in the sample is the sample size.

2.1.11 SAMPLING GUIDE. The part of the surveillance/monitoring plan which contains all the information need to perform a random sample.

SECTION 3

GOVERNMENT FURNISHED PROPERTY

3.1. Water Resources. The Savannah River Site will supply water source.

3.1.2. Site Map. Map of the Savannah River Site will be provided.

SECTION 4

CONTRACTOR FURNISHED PROPERTY/SUPPLIES/MATERIALS/EQUIPMENT/ETC.

4.1 General. Except for those items or services specifically stated to be Government furnished in Section 3, the contractor shall furnish everything (equipment, materials, supplies, incidentals, etc.) required to perform the tasks identified in this contract in accordance with Section 6 Publications in a skillful and workmanlike manner.

4.2 Specific Requirements. The items listed below shall meet the standard or characteristic specified.

4.2.1 All equipment proposed for use in the performance of the work shall be of the size, type and in satisfactory operating condition to fully perform each task order. Any movement or transportation of equipment to and from the work areas for repair, replacement, or for Contractor's convenience shall be at the Contractor's expense.

4.2.2 The Contractor shall furnish all fuel, oil, grease, repairs, and pay any other expenses incidental to the operation of the equipment.

SECTION 5

SPECIFIC TASKS

5.1 Task 1 – Furnish appropriate equipment for Calcium Chloride Application.

5.1.1 Quality Standard. All equipment used in the performance of the work shall be of the size, type and in such condition to properly and safely pursue the requirements of the contract.

5.2 Task 2 – Provide government’s suggested rate of coverage or manufacturer’s recommended application rate for Calcium Chloride on the roads specified in the contract/task order.

5.2.1Quality Standard. Upon completion of Calcium Chloride Application, roads shall produce limited dust fly evenly across the entire width of the traveled road surface.

5.3 Task 3 – Provide proof of measurement of Calcium Chloride.

5.3.3 Quality Standard. Accurate measuring devices and supply metered tickets for the designated road being treated. Road width and length of application will also be used to validate the amount invoiced.

SECTON 6

PUBLICATIONS AND CITED STANDARDS

Typical commercial acceptable practices: Calcium Chloride (38% brine solution) applied at a rate of 0.3 gallons per square yard (approx. 2400 gallons per mile for a 14 foot wide road, 2800 gallons per mile for a 16 foot wide road, 3100 gallons per mile for an 18 foot wide road and 4900 gallons per mile for a 28 foot wide road) has proved to be successful on the Savannah River Site.
