About this file

This document is an Asbestos Survey Results report for a demolition project located at 57600 McKenzie Hwy, McKenzie Bridge, Oregon, prepared for the United States Forest Service. The survey was conducted by Alpine Abatement Associates on 8/29/2025, with Seth Webb as the certified AHERA inspector. The 15,445 square foot commercial firefighting warehouse, built in 1990, was thoroughly inspected for potential asbestos-containing materials (ACM) using sampling protocols from the Asbestos Hazard Emergency Response Act.

The laboratory analysis by Atlas Labs found no asbestos-containing materials in any of the samples collected from various areas including the conference room, locker room, NW office, engine bay, office hallway, exterior, fire workplace, engine bay storage, and stairs. The report recommends contacting a licensed abatement contractor or the Oregon Department of Environmental Quality for further guidance, and emphasizes that a copy of the survey must be kept onsite during any remediation, renovation, or demolition activities. The TCLP analysis performed by SanAir Technologies also confirmed no significant levels of hazardous metals in the debris samples.

PROJECT NUMBER: O2827

Purpose for Survey

This survey was requested by the said customer to fulfill applicable regulatory compliance both in the

State of Oregon and on a Federal level.

Discussion of Findings

Asbestos Containing Materials:

The EPA, Oregon DEQ, and Oregon-OSHA define ACM as "any material containing more than one percent asbestos". Oregon-OSHA has adopted federal regulation governing asbestos (29 CFR Part

1926.1101). These regulations address work procedures and how asbestos-containing materials are removed. Hazard communication, training, personal protection, work practices, exposure monitoring, and recordkeeping are all major components of the regulation.

Reference documents for the removal of asbestos-containing materials include the following:

1. Oregon Occupational Safety and Health Administration (OAR-437, 1926.1101 Asbestos)

2. Department of Environmental Quality (OAR-340, Division 248)

3. OSHA (OAR 437-003-1926.1101) for projects <1%

Sample ID:

N/A: Accredited Asbestos Abatement Company to follow OAR-340, Division 248 standards

Special Terms & Conditions

No prior inspection by Alpine Abatement Associates Inc. has been performed on the property and all owner specified investigations are to be conducted at the time of the initial survey. A minimum number of samples were taken to ensure full accounting of potential ACM, while keeping sampling and analysis costs at an acceptable level to owner.

Detailed Description of Limitation and Thoroughness of Survey

Asbestos surveys are non-comprehensive by nature and subject to many limitations including those presented. This report applies only to the specific subject property, location and area detailed above.

While areas specified by the customer were surveyed and materials sampled, areas behind walls and/or covered by structural members, or materials requiring destructive means to access which could not be found with reasonable diligence were not sampled during the survey. There can be hidden crawl spaces and cavities that were not surveyed. Any areas not specified to be surveyed cannot be assumed to be free of asbestos as no survey was performed to determine the presence of asbestos-containing materials in these areas. This survey report is not intended for use as an asbestos abatement plan, specifications of work scope, or management of risks associated with asbestos. No part of this survey is intended to be used as renovation, addition or demolition plans and specifications.

Sampling Methodology

This survey was performed using sampling protocols adopted from the Asbestos Hazard Emergency

Response Act (AHERA) and modified to meet modified to meet Oregon DEQ asbestos survey and report requirements found in OAR 340-248-0270.

- Areas in the structure were grouped into homogeneous areas based on use or sameness of building materials used in that area; e.g. wall texture, etc.

- Each specified area was sampled for those materials which were identified as suspect ACM.

- When sampling, all samples were collected to the maximum extent possible (taken to substrate) to ensure all components were present in sample which might include asbestos.

- A minimum of one (1) bulk sample for each homogeneous material suspected of being asbestos-containing material was collected. OAR 340-248-0270(3)(a)

- A minimum of three (3) random bulk samples for each homogeneous sprayed or troweled on material were collected. OAR 340-248-0270(3)b)

- Samples were numbered using sequential numbering system to ensure quality control and replication of results for further surveys if needed during demolition of structure.

- Analysis of samples was performed by a laboratory that participates in a nationally recognized accreditation program for asbestos testing, or participates in, and maintains passing status in a nationally recognized bulk sample asbestos proficiency testing program. OAR 340-248-

0270(3)(c)
