About this file
This file is a Past Performance Questionnaire template designed to evaluate offerors' historical contract performance as part of the proposal evaluation process for foreclosure legal services and technical support in North Carolina, South Carolina, and Arkansas. The questionnaire is completed by references (previous clients) rather than the offeror themselves and assesses performance across multiple dimensions using a five-point rating scale: Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory.
The questionnaire evaluates two primary performance categories: Timeliness of Performance and Quality Control/Customer Satisfaction. Under timeliness, references rate the firm's resource utilization, personnel acquisition and training, workforce retention, schedule planning, and timely implementation of corrections. Under quality control, references assess the firm's quality assurance procedures, responsiveness to technical direction changes and corrective actions, and overall professionalism, courtesy, flexibility, and responsiveness. The template requires references to provide detailed comments for any Marginal or Unsatisfactory ratings and includes a final yes/no question regarding willingness to do business with the firm again. The questionnaire captures offeror information (company name, contract number, period of performance, contract value, and contract type) and reference information (company name, point of contact, address, telephone, email, completion date, and signature).
