The form is designed for vendors bidding on contracts exceeding $10,000, requiring certification of compliance with specific federal tax and criminal history requirements.

The certification mandates that bidders affirm two key conditions: (1) they have no outstanding federal tax liability that has been assessed and for which all judicial and administrative remedies have been exhausted, and (2) they have not been convicted of a federal felony criminal violation within the preceding 24 months. The form applies to all business types including corporations, partnerships, trusts, joint-stock companies, sole proprietorships, and other business associations. Vendors must complete the form by providing their company name, date, signature, and title to verify their compliance with these federal procurement standards.
