About this file
This is a Disclosure of Certain Relationships notice issued by the City of San Angelo, Texas Purchasing Division regarding compliance requirements for vendors considering business with the City of San Angelo or the City of San Angelo Development Corporation. Effective January 1, 2006, Chapter 176 of the Texas Local Government Code requires vendors to disclose any affiliations or business relationships that might create conflicts of interest. Vendors responding to Requests for Proposals, Requests for Bids, or Requests for Qualifications from either entity must complete the Form CIQ questionnaire if applicable. The document identifies Local Government Officers of the City of San Angelo as defined by Chapter 176, including City Council members (Mayor Tom Thompson and six council members representing different single-member districts), City Manager Daniel Valenzuela, and Development Corporation officers (Rick Mantooth as President, Steven Mahaffey as First Vice President, Carlos Rodriguez as Second Vice President, and five additional directors, with Ryan Gaddy serving as Executive Director).
The Form CIQ, if required, must be filed with the City Clerk not later than the 7th business day after the vendor becomes aware of facts requiring disclosure. By submitting any response to a City of San Angelo or Development Corporation solicitation or by conducting business with either entity, vendors represent compliance with Chapter 176 requirements. Violation of these provisions is classified as a Class C misdemeanor. Additional information regarding the provisions of Chapter 176 and the Form CIQ questionnaire is available on the Texas Ethics Commission website, and a current list of City officers is available through the City Clerk's office or the City's website.
