About this file
PUR 1355 is a Foreign Country of Concern Attestation form required by the State of Florida under Rule 60A-1.020, F.A.C., applicable to entities submitting bids or proposals to the Florida Department of Juvenile Justice for RFP #10991, the Delinquency-Dependency Case Management Pilot. The form mandates that responding organizations certify they are not owned by, organized under the laws of, or have their principal place of business in a Foreign Country of Concern, and that no Foreign Country of Concern government holds a controlling interest in the entity. The attestation must be completed and signed by an officer or representative of the bidding entity under penalties of perjury.
This attestation form is a compliance requirement for the Miami-Dade County pilot program procurement, which seeks a non-profit organization to provide comprehensive case management services for youth involved in both delinquency and dependency systems from January 1, 2026, through December 31, 2030. The form serves as part of the entity's eligibility verification process and must be included with the proposal submission by the January 1, 2026, deadline. Failure to complete and submit this attestation accurately constitutes a material deficiency in the proposal submission.
