About this file

This is a mandatory attachment form for Request for Proposal #10925 issued by the Florida Department of Juvenile Justice (DJJ) and the Office of Probation and Community Intervention for Mental Health and Substance Abuse Evaluation and Treatment Services. Respondents must complete Attachment I to propose at least one treatment modality for each judicial circuit in which they seek to provide services; different modalities may be proposed across circuits. The form requires detailed specification of each proposed modality including service type classification (Mental Health, Substance Abuse, or Integrated), modality name, publisher/developer information, specific risk factors or needs addressed, appropriate age ranges for youth up to age 21, minimum staff qualifications and training requirements, delivery frequency, individual session length not exceeding 90 minutes, total treatment duration, session format (Individual, Family, and/or Group), and whether the modality is recognized by the Department as a clinical treatment intervention. Respondents must identify all proposed modalities in the corresponding Price Sheet (Attachment J), and any modality not currently recognized on the Department's Research Informed and Best Practices webpage requires prior Department approval before inclusion in any resulting rate agreement.

The form serves as a critical compliance and planning document that ensures providers align their service offerings with departmental standards and enables transparent pricing and program evaluation. Each modality row must correspond to pricing information and implementation details provided elsewhere in the technical proposal. The mandatory nature of this attachment and its integration with pricing documentation indicates that treatment modality selection directly impacts contract feasibility, staffing requirements, and financial projections for respondents bidding on this statewide procurement across Florida's 20 judicial circuits.
