About this file

This document is Attachment I for Request for Proposal (RFP) #10925 issued by the Florida Department of Juvenile Justice (DJJ) and the Office of Probation and Community Intervention, focusing on a mandatory Treatment Modality form for Mental Health and Substance Abuse Evaluation and Treatment Services. The form requires respondents to propose at least one treatment modality for each judicial circuit, with detailed specifications including service type, modality name, publisher, risk factors addressed, appropriate youth ages, staff qualifications, session frequency, session length, duration, session type, and DJJ recognition status. Respondents must complete this attachment for each proposed modality and include corresponding pricing in Attachment J, with modalities not currently recognized requiring prior departmental approval.

The Treatment Modality form provides a comprehensive template for providers to outline their proposed clinical interventions, emphasizing flexibility in approach while maintaining strict documentation and qualification standards. The form aligns with the broader RFP's goals of delivering mental health and substance abuse services to youth aged 12-19 under DJJ supervision, with an anticipated contract period from January 1, 2026 to December 31, 2029. The document underscores the department's commitment to evidence-based practices by requiring modalities to be either currently recognized on their Research Informed and Best Practices webpage or subject to explicit approval, thus ensuring high-quality, clinically validated treatment approaches for juvenile rehabilitation and intervention services.
