Automatic Lighting Controls Maintenance and Services

Companies shall complete and submit the forms below as part of their response to this RFP. Additional pages may be attached as needed to present the information requested.

1. Check all products your firm is certified/factory authorized to work on.

2. Are you able to purchase repair/service parts and equipment for the City’s Automatic Lighting control systems? Check all that apply.

3. List the number of dedicated controls technicians on staff qualified to work on controls.

4. When was your company established? How many years have you been in business providing Automatic Lighting controls service and maintenance?

5. Describe your organization’s capabilities/ knowledge as they relate to Automatic Lighting control systems.

6. How are emergency situations handled? Are you able to provide a controls technician that will respond onsite within 2 hours for emergency service requests 24-hours per day/365 days per year?

7. Describe components of the maintenance program.

8. Provide a list of your materials suppliers.

9. Have you been involved in any litigation in the past 5 years? If yes, please provide an explanation.
