About this file
This is a Form 1295 Acknowledgment of Exemptions form issued by Fort Bend Independent School District (FBISD) in Texas, required under House Bill 1295 and Texas Government Code Section 2252.908. The form pertains to the Course Registration and Evaluation Management System RFP, a multi-category solicitation in which vendors may propose for Category 1 (Course Registration System), Category 2 (Evaluation Management System), or both categories through separate submissions. FBISD requires all business entities to submit a completed Disclosure of Interested Parties (Form 1295) before executing any Board- or Superintendent-approved contract, unless the contract qualifies for specific exemptions.
The form identifies six categories of contracts exempt from the Form 1295 requirement: sponsored research contracts of institutions of higher education, interagency contracts of state agencies or institutions of higher education, health and human services contracts where value cannot be determined at execution and any qualified vendor is eligible, contracts with publicly traded business entities or their wholly owned subsidiaries, contracts with electric utilities as defined by the Texas Utilities Code Section 31.002, and contracts with gas utilities as defined by Texas Utilities Code Section 121.001. Vendors must certify the accuracy of information provided and understand that failure to comply with Texas Government Code 2252.908 may result in contract delays or termination. The form instructs vendors to check applicable exemption boxes, obtain an authorized signature, and upload the completed form under the HB 1295 Form section in the Required Forms area of the Bonfire procurement system.
