About this file
This document is a Felony Conviction Notification form from the Amarillo Independent School District in Texas, related to Texas Legislative Senate Bill No. 1, Section 44.034, regarding Notification of Criminal History. The form requires businesses or individuals entering into a contract with the school district to disclose any felony convictions of owners or operators.
The form provides three checkbox options: certifying the firm is a publicly-held corporation (exempt from reporting), confirming the firm is not owned or operated by anyone with a felony conviction, or disclosing specific felony convictions. If a firm fails to provide required notification or misrepresents conviction details, the school district may terminate the contract while still compensating the vendor for services performed prior to termination. The document emphasizes transparency and allows the school district to make informed decisions about potential contractors based on criminal history.
