About this file

This is an insurance requirements document issued by the City of Lewisville, Texas, establishing mandatory coverage parameters for vendors providing services under city contracts. The document applies to various service categories including janitorial services, maintenance agreements, electrical work, plumbing, welding, painting, and related trades. Vendors must procure and maintain comprehensive insurance coverage throughout the contract duration, with all associated costs incorporated into their bids. The city requires Commercial General Liability coverage under an occurrence-based form (CG 00 01), Workers' Compensation insurance compliant with Texas Labor Code requirements, and Automobile Liability coverage for any vehicles used in contract performance. No specific project quantities, response dates, or contract term lengths are identified in this document, as it serves as a standard requirements exhibit applicable across multiple contract opportunities.

Minimum insurance limits mandate $500,000 per occurrence and $1,000,000 aggregate for Commercial General Liability, statutory Workers' Compensation limits with Employer's Liability minimums of $500,000 per injury, occurrence, and occupational disease, and $500,000 Combined Single Limit for Automobile Liability, with reductions only available through HR Director approval. The City must be named as an additional insured with primary and non-contributory coverage, and all policies require waiver of subrogation provisions and thirty-day cancellation notice requirements. Insurers must maintain A.M. Best ratings of A-:VI or better, or Standard and Poor's ratings of A or better. Vendors must provide certificates of insurance on Texas Department of Insurance-approved forms prior to contract execution, with the city retaining the right to request complete certified policy copies. The document includes comprehensive indemnification and hold-harmless provisions requiring contractors to defend and indemnify the city against claims arising from contractor negligence or breach, except where claims result solely from city negligence.
