About this file

This is a Drug-Free Workplace Certification Form required by Hillsborough County, Florida in accordance with Florida Statute 287.087 for the Fire Program Construction Manager at Risk (CMAR) solicitation. The form serves as a mandatory compliance document that vendors must complete and submit as part of their qualification proposals. The certification requires the undersigned vendor to confirm implementation of a comprehensive drug-free workplace program, including publication of workplace conduct policies prohibiting controlled substance use, employee education regarding drug abuse dangers and company policies, distribution of policy statements to all employees engaged in the contract work, employee notification of reporting requirements for drug-related convictions within five days, and imposition of sanctions or mandatory participation in rehabilitation programs for violations. Vendors must also demonstrate good faith efforts to maintain drug-free workplace compliance throughout the contract term.

The form functions as a pre-award requirement with no specific pricing information contained within the document itself. Completion and signature by an authorized company representative certifies full compliance with all six statutory requirements outlined in Florida Statute 287.087. The certification applies to all employees providing commodities or contractual services under the Fire Program CMAR bid, which involves qualification proposals from firms with proven experience in construction management at risk services for upcoming fire program projects in Hillsborough County with an estimated project value of $2,000,000.00 and a Small Business Enterprise (SBE) goal to be established upon design completion at 60-90% phase.
