About this file

This is a Domestic Partnership Act Certification form for Broward County, Florida, applicable to vendors responding to the Safety Footwear Services solicitation. The certification is a mandatory responsiveness requirement for all contracts exceeding $100,000 in value. Vendors must complete and return the form with their solicitation submittal or within three business days of the County's request; failure to comply within the stated timeframes renders the vendor nonresponsive to the solicitation. The form requires vendors to certify their compliance status with Broward County's Domestic Partnership Act (Sections 16-150 through 16-165, Broward County Code of Ordinances), which mandates that vendors provide benefits to registered domestic partners of employees on the same basis as spousal benefits.

Vendors must select one of three compliance options: current compliance with the Domestic Partnership Act requirements, commitment to comply at contract award, or non-compliance. Alternatively, vendors may qualify for exemptions if they employ fewer than five employees, do not provide spousal benefits, are governmental entities or not-for-profit organizations, are religious institutions, provide cash equivalents of benefits, or cannot comply due to federal or state law restrictions or grant conditions. The form, dated September 9, 2024, requires vendor name, authorized signature, printed name, title, and date of certification. Submission of this completed certification is critical to vendor eligibility and responsiveness in the procurement process for safety footwear services across Broward County agencies and the Transportation Department.
