About this file
This is an Invitation for Bids (IFB) issued by the Utah Department of Health and Human Services for optometry services at Utah State Hospital under solicitation number DHHS26065. The procurement seeks a single five-year contract with a qualified optometrist to provide optometry services. Bids must be submitted electronically through Utah Public Procurement (U3P) by the deadline published on that platform. Bidders must meet four mandatory minimum requirements: maintain a current and valid Utah optometry license, hold a doctorate of optometry degree, possess a minimum of five years of experience as a licensed optometrist, and have at least one year of direct experience working with mentally ill patients. The IFB allows for a question and answer period during which bidders may seek clarification and propose exceptions to terms and conditions; all answers will be posted on U3P. Bidders are prohibited from communicating with DHHS employees regarding the solicitation prior to award.
Evaluation of bids will be based on responsiveness to stated requirements, with the lowest responsive and responsible bid selected for award. Bidders must submit a completed cost form; failure to do so will result in bid rejection. The selected contractor must guarantee pricing for the entire five-year contract term, with any price adjustments requiring at least 30 days' advance notice, supporting documentation, and approval by the DHHS Director. Contracted support coordinators are ineligible to bid unless their existing contracts are terminated. All work must commence only after contract award and execution; DHHS assumes no liability for work performed or costs incurred prior to official contract signing. Bidders must accept all final terms and conditions as published; bids containing exceptions or additions to standard terms will be rejected and disqualified.
