About this file
This is a Limited Hazardous Materials Inspection Report prepared by Terracon Consultants, Inc. for the State of Utah Division of Facilities Construction and Management (DFCM) regarding the Brigham Regional Center located at 325 West 1100 South, Brigham City, Utah. The inspection was conducted on September 25, 2025, to identify asbestos-containing materials (ACM), lead-based paint, and universal hazardous materials in two electrical rooms—an upstairs electrical room (approximately 256 square feet) and a downstairs electrical closet (approximately 27 square feet)—in preparation for electrical panel replacement and energy controller removal work. Terracon collected 15 bulk samples from five homogeneous areas of suspected ACM and tested all accessible building components for lead using X-ray Fluorescence (XRF) analysis. The inspection included visual assessment of friability conditions and documentation of hazardous waste materials requiring proper handling or disposal if renovation activities warrant removal.
The inspection findings indicate no asbestos was detected in any of the 15 samples analyzed by Eurofins EMLab P&K using polarized light microscopy (PLM) and EPA Method 600/R-93-116. Lead was detected at 0.04 mg/cm² on the paint of the upstairs electrical room metal door, which exceeds the threshold of 0.01 mg/cm² and requires compliance with OSHA Lead in Construction Standard 1926.62. Two possible mercury-containing thermostats and four fluorescent light tubes with ballasts were identified in the electrical rooms and may require proper disposal under EPA Title 40 regulations if disturbed during construction activities. The report notes that additional suspect materials may exist in concealed areas such as walls or voids and recommends that contractors review findings independently. This inspection supports the concurrent DFCM construction project (#26353310) for exterior disconnects and electrical upgrades at Brigham Regional Center, estimated at $122,000.

Dear Ms. Stapel:
The purpose of this report is to present the results of the limited hazardous materials inspection on September 25, 2025, at the Brigham Regional Center. We understand that this inspection was requested due to the replacement of electrical panel and removal of the energy controller within the electrical closet on the main floor adjacent to the medical training lab and the electrical room on the second floor above. The purpose of this work was to identify and quantify any suspect asbestos containing materials (ACM), assess painted surfaces for lead, and catalog universal hazardous materials in or on these electrical areas.
Based on laboratory analysis, no asbestos-containing materials were identified during the inspection. Lead was detected on the door of the upstairs electrical room. Two possible mercury containing thermostats were identified and four 4-foot light tubes and ballasts were identified in the specified rooms; however, if the nature of the work does not include disturbance or disposal of these, they may remain in place. 1.0 Introduction
Terracon Consultants, Inc. (Terracon) conducted a limited hazardous materials inspection including asbestos, lead, and other hazardous materials, in two electrical rooms located within the Brigham Regional Center building located at 325 West 1100 South, Brigham City, Utah.
The inspection was conducted on September 25, 2025, by Mr. Grant Coleman, a Terracon employee and a State of Utah-certified Asbestos Inspector. He is also trained to inspect for and summarize the presence of lead and universal hazardous wastes for proper disposal as required by the United States Environmental Protection Agency (EPA).
1.1 Project Objective
The objective of this project was to identify the presence of asbestos, lead, universal hazardous waste, and other hazardous materials present in the two electrical rooms.
2.0 Building/Area Description
The upstairs electrical room located in the southwest area of the building is approximately 256 square feet, with a concrete floor, gypsum drywall, concrete masonry unit (CMU) block, and oriented strand board (OSB) walls, with an exposed beam ceiling. The downstairs electrical closet is also located in the southwest area of the building. It is located within the medical training lab. This closet is approximately 27 square feet, consisting of a concrete floor, gypsum drywall walls, and exposed chase ceiling.
3.0 Field Activities
The limited hazardous materials inspection, including asbestos, lead, and other hazardous materials for the electrical rooms, was conducted on September 25, 2025, by Mr. Grant Coleman, a Terracon employee and State of Utah-certified Asbestos Inspector. He is also trained to inspect for and summarize the presence of lead and universal hazardous wastes for proper disposal as required by the United States Environmental Protection Agency (EPA).
Copies of the inspector’s Asbestos Inspector certifications are attached as Appendix F.
3.1 Asbestos
Terracon began the asbestos sampling activities with a visual assessment and the identification and inventory of readily visible and accessible homogeneous areas of suspect ACM. A homogeneous area consists of a material that appears similar throughout in terms of color, texture, and date of application. The assessment was conducted throughout
Brigham Regional Center| Brigham City, Utah October 8, 2025 | Terracon Project No. 61257226
Facilities | Environmental | Geotechnical | Materials 2 visually accessible areas of the buildings. Materials identified as glass, metal, plastic, or rubber were not considered suspected ACM.
A physical assessment of each homogeneous area of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the EPA as a material that can be crumbled, pulverized, or reduced to powder by hand pressure when dry. Friability was assessed by physically touching suspect material.
Based on results of the visual observation, bulk samples of suspect ACM were collected from the electrical rooms. Bulk samples of suspected ACM were collected in accordance with AHERA sampling protocols, using wet methods as applicable to reduce the potential for fiber release. Samples were placed in sealable containers and labeled with unique sample numbers, using an indelible marker. Terracon collected 15 samples from five homogeneous areas of suspect ACM. A sampling location drawing is provided in Appendix B. Descriptions of the suspect materials, sample locations, and laboratory results are summarized in Table 1 Appendix A.
Samples of suspect ACM were delivered under standard chain-of-custody protocol to Eurofins EMLab P&K (NVLAP #500031-0) in Phoenix, Arizona, for analysis by PLM, using EPA Method 600/R-93/116. The asbestos content, where applicable, was determined by microscopic visual estimation. Copies of the analytical reports and chain-of-custody forms are provided in Appendix C.
3.2 Lead
The lead paint inspection began by visually surveying the building components, such as doors, door frames, drywall, and electrical panel. Various colors of paint were found on the surfaces. All these components were tested for lead.
Terracon directly measured the concentration of lead using an Olympus X-ray Fluorescence (XRF) Lead Analyzer (Serial Number 510179). The Olympus XRF nondestructively measures lead concentrations in coated surfaces, regardless of the number of paint layers present. For this survey, materials were considered lead containing if the XRF indicates readings greater than or equal to 0.01 mg/cm2; however, since OSHA does not permit materials to be classified as lead-free using an XRF, Terracon cannot guarantee that lead is not present in very low concentrations in these materials.
3.3 Hazardous Materials
Terracon visually inspected for hazardous waste that would require removal and proper disposal prior to renovation or demolition, if the nature of replacing the electrical panels warrants their removal.

4.0 Regulatory Overview
4.1 Asbestos
The Utah Division of Air Quality (DAQ) requires buildings with asbestos surveys greater than 3 years old to have a reinspection conducted of at least the areas of planned renovation.
The DAQ enforces 40 CFR Part 61, Subpart M, the Asbestos National Emission Standards for Hazardous Air Pollutants (NESHAP) adopted by reference in Section 214-1, Utah Air Conservation Rules. The owner or operator must provide DAQ with written notification at least 10 working days prior to the commencement of asbestos abatement activities that will disturb regulated asbestos-containing material (RACM) in amounts greater than or equal to 3 square feet or 3 linear feet.
The asbestos NESHAP regulates asbestos fiber emissions and asbestos waste disposal practices, and requires that RACM be identified, classified, and quantified prior to planned disturbances or demolition activities. The asbestos NESHAP regulation also requires the identification and classification of existing ACM according to friability prior to renovation or demolition activity. Friable ACM is a material containing more than 1% asbestos that, when dry, can be crumbled, pulverized, or reduced to powder by hand pressure. All friable ACM is considered RACM.
The asbestos NESHAP regulation classifies ACM as either RACM, Category I non-friable ACM, or Category II non-friable ACM. RACM includes all friable ACM; Category I non-friable ACM that has become friable or will be or has been subjected to sanding, grinding, cutting, or abrading; and Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder in the course of demolition or demolition activity. Category I non-friable ACM are exclusively asbestos-containing packings, gaskets, resilient floor coverings, resilient floor covering mastics, and asphalt roofing products that contain more than 1% asbestos. Category II non-friable ACM are all other non-friable materials other than Category I non-friable ACM that contain more than 1% asbestos. Category II non-friable ACM generally includes but is not limited to glazing, mortar, grouts, and cementitious material such as pipes, siding, and panels.
The OSHA asbestos standard for construction (29 CFR 1926.1101) regulates workplace exposure to asbestos. The OSHA standard classifies construction and maintenance activities that could disturb ACM and specifies work practices and precautions that employers must follow when engaging in each class of regulated work. The standard also specifies requirements for disturbing and handling materials with asbestos concentrations less than or equal to one percent.

Facilities | Environmental | Geotechnical | Materials 4
4.2 Lead Paint
The Lead in Construction OSHA Standard 1926.62 regulates construction work involving the presence of lead in any concentration. Depending on the specific construction activities being performed (torch cutting, manual demolition, sanding, grinding, etc.), employers must assume their employees are exposed to lead above the occupational exposure limit, and must provide personal protective equipment, including respiratory protection, until an exposure assessment proves otherwise.
The presence of lead in demolition debris from non-residential buildings has the potential to impose limitations on where and how the debris may be disposed. The Resource Conservation and Recovery Act (RCRA) requires each waste generator to determine if wastes are hazardous. This can be determined either through generator knowledge or by testing. Toxicity Characteristic Leaching Procedure (TCLP) testing is the required method for determining if wastes are hazardous with respect to lead content. A composite sample representative of the waste stream from this project should undergo TCLP testing prior to disposal to determine if the waste stream is likely to be hazardous with respect to lead content.
4.3 Hazardous Materials
US EPA (Title 40) requires the following materials to be identified, quantified, removed, and properly disposed of/or recycled prior to renovation or demolition of the building.
mercury-containing thermostats mercury-containing fluorescent lights polychlorinated biphenyls (PCB)-containing ballasts or transformers refrigeration units containing chlorofluorocarbons (CFCs) containers of liquid or hazardous waste
There were two possible mercury containing thermostats and three fluorescent lights and ballasts found in the upstairs electrical room, and one fluorescent light and ballast found in the main floor electrical closet. These materials were in areas adjacent to the work being performed and will need to be handled appropriately should the nature of the work raise the need to displace or dispose of them.
5.0 Findings
5.1 Asbestos
Terracon collected 15 samples from five homogeneous areas of suspect ACM. Based on the laboratory analytical reporting, no asbestos was detected.
October 8, 2025 | Terracon Project No. 61257226
Facilities | Environmental | Geotechnical | Materials 5
Material descriptions, locations, and sample results are summarized in Table 1: Asbestos Bulk Sampling Summary in Appendix A. Laboratory analytical reports are included in Appendix C. A drawing depicting sample locations is provided in Appendix B.
5.2 Lead Paint
Lead was detected at a concentration of 0.04 mg/cm2 in the paint on the upstairs electrical room metal door. Results of surface coatings tested for lead are provided in Appendix E.
5.3 Hazardous Materials
Hazardous materials were identified adjacent to the work area. The two thermostats that possibly contain mercury, and the fluorescent light and ballasts will need to be handled accordingly if they are to be disposed of.
5.4 Inaccessible Areas
Although reasonable effort was made to inspect accessible suspect materials, additional suspect but unsampled materials could be in walls, in voids, or in other concealed areas.
6.0 General Comments

All components not quantified as asbestos content and non-asbestos content are considered to be non-fibrous matrix components. Matrix components may include, but are not limited to, gypsum, paint, silicate minerals, vinyl, binder, calcium carbonate, tar, and foam.
Inhomogeneous samples are separated into homogeneous subsamples and analyzed individually. ND means no fibers were detected. When detected, the minimum detection and reporting limit is less than 1% unless point counting is performed. Floor tile samples may contain large amounts of interference material and it is recommended that the sample be analyzed by gravimetric point count analysis to lower the detection limit and to aid in asbestos identification.
