About this file

This is an Appendix H template document for project experience and references required as part of a proposal response for an Infrastructure Resiliency contract opportunity. Respondents must submit detailed information for their four most recent projects, or fewer if additional projects are unavailable. The template requires documentation of each project using standardized sections or equivalently organized formats. All required elements must be addressed in the proposal submission to enable the State to evaluate offeror qualifications, past performance, and capacity to successfully deliver the required services.

For each of the four projects, respondents must provide comprehensive documentation including client name and contact information, project start and completion dates, estimated versus actual timelines with explanations for variances, detailed project descriptions and goals, the offeror's specific role and responsibilities, accomplishments achieved, project management methodology, estimated versus actual costs with explanations for overruns, descriptions of special constraints and proven ability to manage them, all requested changes to the original plan with completion status, change management procedures, litigation or adverse contract actions if applicable, client feedback on work performance, and written permission for the State to contact clients and obtain references. This information will be used by the State to assess the offeror's experience, performance history, and capacity to meet Infrastructure Resiliency project expectations.

Appendix H:

Project Experience and References

This template is provided for the convenience of RFP respondents. Respondents may use this template or submit the required information in their own format.

Project experience and client references are required as part of the offeror’s proposal. Offerors must provide detailed information for their four most recent projects—or as many projects as available if fewer than four can be provided. These project examples will be used to evaluate the offeror’s qualifications, past performance, and ability to successfully deliver the required services.

Each project must be documented using the templates in this appendix or an equivalently organized document. All required elements must be addressed, including client information, project scope, timelines, costs, deliverables, constraints, change management, and client feedback. The State will use this information to assess the offeror’s experience, performance history, and capacity to meet the expectations of the Infrastructure Resiliency project.
