About this file

This is an Appendix C document from the Florida Department of Financial Services' Criminal Investigations Division outlining the Contract Disputes Form requirements for an Invitation to Bid (ITB) for law enforcement vehicle upfitting services. Respondents are required to disclose any contract disputes occurring within the three-year period prior to submission, including circumstances where they or their affiliates, subcontractors, or agents were identified as being in default or breach, received notices of default or breach, were subject to judicial or quasi-judicial action, or were assessed fines or damages. For each dispute reported, respondents must identify the related contract, explain the nature of the dispute, and describe its resolution. The form allows respondents to indicate either that no contract disputes exist or to report specific disputes, with additional pages available if needed.

The appendix serves as a disclosure mechanism to assess respondent capability and reliability in performing the requested upfitting services. This requirement applies to contracts under which respondents provided commodities and services in the United States at an organizational or enterprise level that may impact their ability to deliver the services described in the solicitation. The form is standardized as part of the Department's competitive sealed response process administered through the Vendor Information Portal (VIP), ensuring all qualified vendors are evaluated on consistent criteria regarding their contractual performance history.
