About this file

This is an Appendix A Capability Form template used by the FAA to assess vendor qualifications for a battery systems procurement. The form requires interested vendors to demonstrate experience and current capabilities across multiple dimensions of battery supply, installation, maintenance, and support services.

Vendors must document three previous contracts (Section A) and three current contracts (Section B) performed within five years, providing detailed information including contract value, period of performance, customer name, point of contact, and work descriptions demonstrating similar size, scope, and complexity to the Draft SOW requirements. Section C requires vendors to confirm capability across eight service categories: Program Management, COTS Battery System supply, Battery System Delivery and Recycling/Disposal, Battery System Replacement, Battery Monitoring System (BMS) Installation and Training, Battery Depot Support, and Technical Support Services. Section D assesses battery technology capabilities across Vented Lead Acid (VLA), Valve Regulated Lead Acid (VRLA), Nickel-Cadmium systems, BMS, Building Ventilation Modifications for hydrogen control, asbestos remediation, and stationary battery string maintenance. Additional sections require vendors to identify manufacturing lead times for routine and urgent projects, disclose subcontractors and teaming arrangements, confirm service delivery capability across nine FAA regional areas (both CONUS and OCONUS), provide warranty approach and procedures for equipment storage and handling, demonstrate asbestos remediation experience, address equipment availability challenges, and submit site survey and technical support examples. The form also solicits vendor feedback on the Draft SOW and requests evaluation of Lithium-ion battery availability as an alternative technology option.

In order to make the best determination, the FAA requires interested vendors to fill out Appendix A addressing the following and per the instructions below:

· Capability Questions start in Block A.

· Interested vendors may reference the same contract, described in Block A Table 1 and Block B Table 2, (A1, A2, A3, B1, B2, B3) for multiple citations.

· Supporting document attachments are permitted, provided they reference the specific contract, described in Block A Table 1 and Block B Table 2, (A1, A2, A3, B1, B2, B3) citation.

· If work was performed on a specific Task Order, cite both the Task Order number and the Base/Parent Contract number.

This form can be completed on government and/or commercial sector projects. Scope may be represented on one contract/project or be represented on separate contract(s)/ project(s) providing same or similar services; within five (5) years of the issuance of the Market Survey.
