Key dates include: questions due by 04/08/2025 at 12:00 PM EST, proposals due by 04/21/2025 at 12:00 PM EST, and sample labels due by 05/05/2025 at close of business. Good
Performance meets contractual requirements and exceeds some requirements to the Government's benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were

effective.

Satisfactory
Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor were satisfactory.
Marginal
Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor did not/has not yet

identified corrective actions.

Unsatisfactory

2. Please provide narrative explanations for any Marginal, Unsatisfactory or N/A ratings and supplement your evaluation of the contractor’s performance with judgement of others in your organization.

3. The Contractor submitted a proposal to the Library for the provision of custom printed and braille embossed labels, and shipping of boxes. Past performance is of considerable importance to this acquisition. Therefore, input from current or previous customers of the contractor is very important.

We greatly appreciate you taking the time to complete this form. Please provide an honest assessment and return via email to Brenda Kinlay at bkinlay@loc.gov with a copy to Jennifer Zwahlen at jzwa@loc.gov no later than Noon (12:00 PM EST) on April 21, 2025.
