About this file
This is a blank W-9 form (Request for Taxpayer Identification Number and Certification) issued by the Internal Revenue Service as Exhibit D to the Butler Park Multipurpose Artificial Turf contract opportunity solicited by Hall County, Georgia, Board of Commissioners. The form serves as a required document for contractors bidding on the project to convert a natural turf multipurpose field at Butler Park into a modern artificial turf system. Contractors must complete and submit the W-9 form to provide their correct taxpayer identification number (either Social Security Number for individuals or Employer Identification Number for entities) and certify their U.S. person status for tax reporting purposes. The form requires contractors to identify their business structure (individual/sole proprietor, C Corporation, S Corporation, Partnership, Trust/estate, Limited Liability Company, or other entity type) and provide their complete legal name and address as shown on their income tax return.
The W-9 form is a standardized IRS document (Rev. October 2018) that must be completed and signed under penalties of perjury by the contractor or authorized representative. Submission of this form is a mandatory administrative requirement for the contracting process and enables Hall County to properly report payments to contractors on appropriate IRS information returns such as Form 1099-MISC. Contractors should note that the information provided on the W-9 must match exactly with the name and taxpayer identification number used in all other bid submission documents to avoid backup withholding complications and ensure proper contract award processing.
