About this file

This is a proposal cover sheet for a Mini-RFP issued by the Oregon Department of Transportation for Crash Data Safety Consultant Support Services (Mini-RFP/Requisition #S-73000-00016737). The document serves as the submission form for interested firms to respond to the solicitation and requires proposers to provide their firm name, primary contact information, email address, and telephone number. The cover sheet requires proposers to document how they meet any minimum requirements specified in the Mini-RFP document, typically found in Section 2.4.7 of the solicitation materials.

The cover sheet includes mandatory certifications that proposers must acknowledge and attest to, including confirmation of receipt of all addenda, verification of proposal accuracy and truthfulness, confirmation of non-discrimination practices toward minority and women-owned business enterprises, and disclosure of any conflicts of interest. Proposers must certify that they have no undisclosed delinquent debt owed to the State of Oregon, accept all terms and conditions of the Mini-RFP, and possess a written policy preventing sexual harassment and discrimination if the anticipated contract price exceeds $150,000. The authorized representative of the proposing firm must sign and date the cover sheet and check the "APPROVED" checkbox to certify their authority to bind the organization and submit the proposal on its behalf. The document does not specify response deadlines, contract terms, pricing information, or funding amounts within the cover sheet itself.
