ODOT intends to award three separate Price Agreements based on regional categories: Category 1 for ODOT Region 1, Category 2 for ODOT Region 2, and Category 3 for ODOT Regions 3, 4, and 5. Proposers may submit proposals for one or multiple regional categories. The cover sheet requires proposers to provide firm information, contact details, authorized signatories, and confirmation that submission deadlines have been met. Proposers must document minimum qualifications including at least one individual meeting the Secretary of the Interior's Standards and Guidelines for Archeology, at least one individual meeting the SOI standards for Architectural History, and at least two ODOT-Qualified personnel (either currently qualified or anticipated to be qualified prior to Price Agreement execution).
The cover sheet includes comprehensive certification and compliance requirements that proposers must affirm, including representations regarding debarment status, System for Award Management exclusions, conflict of interest disclosures, equal employment opportunity practices, sexual harassment prevention policies for contracts valued at $150,000 or more, and compliance with federal and state regulations including the Oregon False Claims Act and Federal False Claims Act. Proposers must submit required attachments including Attachment C (Key Staff Resumes) and Attachment D (Project Samples for each regional category selected). The document emphasizes that proposals are public records subject to Oregon Public Records Law and that proposers bear all costs associated with proposal preparation and submission. Authorized representatives must certify the accuracy and truthfulness of all proposal contents and attest to independent preparation without collusion or fraud.
