About this file
This document is a Notice of Intent to Award from the Oregon Department of Transportation (ODOT) Procurement Office for RFP S-73000-00013855 for Audiometric Services. Listen Audiology Services, Inc. has been selected as the top-ranked firm for contract negotiations to provide Personal Services, with the award process detailed in a comprehensive timeline. The notice outlines a structured negotiation process involving establishing a statement of work (SOW), completing a Breakdown of Costs (BOC), and securing necessary approvals, with specific timelines for each stage ranging from 3-20 business days depending on project complexity.
The document emphasizes that no work is authorized until the contract is fully executed and a written Notice to Proceed (NTP) is issued by the ODOT Project Manager. The procurement includes requirements for submitting various documents such as Billing Rate Information, Certificates of Insurance, Responsibility Inquiry form, Business Registry Number, and W-9 Form. Additionally, ODOT encourages the use of direct deposit via Automated Clearing House (ACH) for consultant payments and suggests that consultants make reasonable efforts to offer ACH direct deposit payments to their subcontractors. The notice also notes that if negotiations cannot be completed within the specified timeframe, ODOT may terminate discussions and begin negotiations with the next highest-ranked firm.

Direct Deposit via Automated Clearing House (ACH). For Consultants that receive ongoing monthly payments from Agency on one or more contracts or work orders, Agency prefers making Consultant payments via ACH direct deposit. While not required, if Consultant has not previously authorized ACH direct deposit and would prefer ACH to expedite transfer of payments after invoices are approved, Consultant must complete a Vendor ACH Authorization form and submit to Agency per the instructions on the form. To expedite Consultant’s payments to its subcontractors, if applicable, Consultant should make reasonable efforts to offer ACH direct deposit payments.
