About this file

This document is a Proposal Cover Sheet for a Mini-RFP/Solicitation #00010997 issued by the Oregon Department of Transportation (ODOT) for the Sisters/Bend/Redmond Active Transportation Plan. The cover sheet serves as a formal certification document for potential proposers, outlining key requirements and commitments for participating in the solicitation process.

The cover sheet requires proposers to certify multiple compliance points, including receipt of addenda, non-discrimination practices, sexual harassment prevention policies, absence of delinquent state debt, and conflict of interest disclosures. Notably, for contracts with an anticipated price of $150,000 or more, proposers must have a written policy preventing sexual harassment and discrimination. The document also requires proposers to accept all terms and conditions of the Mini-RFP and provide an authorized representative's signature and approval, affirming their commitment to the truthfulness and accuracy of their submission.
