About this program

The Department of Health and Human Services Health Resources and Services Administration funds rural healthcare organizational development, quality improvement, and workforce training across three geographically defined regions—the Delta Region, Appalachian Region, and Northern Border Region—under Assistance Listing 93.619, with the most recent award dated September 1, 2026.

The program's portfolio spans ten distinct funding vehicles, all structured as Project Grants. Delta Region initiatives dominate recent activity: the Delta Health Systems Implementation Program funds rural hospitals and health clinics implementing revenue cycle optimization, electronic health record deployment, chronic care management, telehealth infrastructure, and workforce retention strategies. The Delta Region Community Health Systems Development Program provides multi-year technical assistance to rural healthcare organizations addressing financial and operational performance. Delta Region Maternal Care Coordination supports perinatal care access and coordination across eight Delta states. Northern Border Region programs fund healthcare delivery expansion through network development, outreach coordination, and integrated care planning, with recent awards supporting dental access for children in Maine and rural obstetrics training. The Rural Communities Opioid Response Program funds behavioral health workforce development and substance use disorder services in the Northern Border Region. An Appalachian Region Healthcare Support Program provides technical assistance to critical access hospitals and small rural hospitals in Appalachian counties.

The program has disbursed $370.7 million in federal dollars across 37 awards since 2017, with significant acceleration: only 2 awards in 2017–2018, then 17 in 2024, 8 in 2025, and 10 in 2026 year-to-date. Combined federal and non-federal obligated dollars total $381.5 million. All funding originates from HRSA. Recent award magnitudes typically range from $350 thousand to $2.7 million for operational and implementation projects; technical assistance and workforce programs carry larger obligations, with the Rural Communities Opioid Response Technical Assistance award at $177.8 million and Delta Region Community Health Systems Development at $151.1 million. Representative recent awards include St. Bernards Five Rivers Medical Center in Arkansas ($350 thousand, September 2026) implementing revenue cycle best practices; Ste. Genevieve County Memorial Hospital in Missouri ($350 thousand, September 2026) addressing employee engagement and retention; and Pemiscot Memorial Health Systems in Missouri ($700 thousand, September 2026) modernizing EHR systems, cybersecurity infrastructure, and diagnostic capabilities.

Multiple opportunities remain open or recently closed: Delta States Rural Development Network Program (HRSA-26-045, due August 12, 2026, now closed) solicited integrated healthcare network planning and implementation across eight Delta states; Rural Northern Border Region Network Planning Program (HRSA-26-100, due July 10, 2026, now closed) supported one-year network development; Delta Rural Integrated Health Network Program (HRSA-26-073, due July 17, 2026, now closed) funded integrated network development among rural hospitals, primary care clinics, and behavioral health providers. The Appalachian Region Healthcare Support Program (HRSA-27-053, posted August 24, 2026) is currently open as a Cooperative Agreement, accepting applications from states, nonprofits, tribal governments, and other entities to provide technical assistance to rural healthcare organizations in Appalachian counties.
