About this opportunity

The Health Resources and Services Administration seeks applicants for the Delta States Rural Development Network Program, a $12 million grant initiative with an application deadline of August 12, 2026 at 11:59 p.m. ET. HRSA expects to make 12 awards ranging from $905,664 to $1,188,684 over three 12-month budget periods (September 30, 2026 through September 29, 2029), with up to $56,604 available per required eligible county or parish per year.

Eligible applicants include domestic public or private, nonprofit or for-profit entities, including faith-based and community-based organizations, federally recognized tribes, and tribal organizations with demonstrated experience or capacity to serve rural populations within the Delta region (Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee). Applicant organizations must represent a health care network of at least three separately owned network participants with their own EIN/UEI numbers, with at least 66 percent of network participants physically located in HRSA-designated rural areas within the Delta region. All activities must exclusively target populations in rural areas and serve only counties or census tracts designated as HRSA-designated rural areas.

The program supports planning, development, and implementation of integrated health care networks that achieve efficiencies, expand access to and improve quality of basic health care services, and strengthen the rural health care system. Applicants must propose projects based on no more than two focus areas from the following: diabetes, cardiovascular disease, obesity, acute ischemic stroke, chronic lower respiratory disease, cancer, or unintentional injury and substance use. Programs must utilize evidence-based or promising practice models and may incorporate value-based care approaches. Recipients must consult with their State Office of Rural Health and submit work plans, service area maps, staffing plans, biographical sketches, memorandums of understanding from network participants, and network organizational charts. The program aligns with Making American Health Again priorities.
