About this opportunity
The Health Resources and Services Administration is soliciting applications for the Delta Rural Integrated Health Network Program to support planning and development of integrated healthcare networks in the Delta Region, with applications due July 17, 2026. Eligible applicants include domestic public entities, nonprofits, for-profit organizations, state and local governments, tribal entities, and institutions of higher education that represent networks of at least three healthcare organizations with at least 66 percent physically located in HRSA-designated rural areas within the Delta region spanning 255 counties and parishes across Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee. No cost-sharing is required.
HRSA will award eight grants totaling $2.8 million, with individual awards up to $350,000 for a two-year period of performance from September 30, 2026 to September 29, 2028. Applicants must select one of two funding tracks: Track 1 for creating new networks or Track 2 for strengthening existing networks. Eligible planning activities include rural healthcare network integration, strengthening operations, creating care coordination policies, convening participants for strategic planning, and contracting for facilitation, legal, and financial feasibility services. Prohibited uses include direct clinical service delivery, real property acquisition, construction, and unrelated equipment costs.
Merit review evaluates applications on six criteria totaling 100 points: Need (15 points), Response/Approach (30 points), Performance Reporting and Evaluation (15 points), Impact (10 points), Resources and Capabilities (15 points), and Support Requested (15 points). Three funding preferences apply for organizations located in Health Professional Shortage Areas, Medically Underserved Communities or Populations, or those focusing on primary care and wellness prevention strategies. Required attachments include work plans, staffing plans, biographical sketches, network agreements, organizational charts, participant information, and letters of commitment from all network participants. Applications are submitted through Grants.gov. The assistance listing is 93.619.
