Contractors must disclose any OSHA violations or safety-related contract terminations with public entities within the three years prior to bid submission; those cited for willful violations, violations for failure to abate, repeated violations, or three or more serious violations are generally prohibited from bidding unless they meet eligibility criteria established by the County Risk Manager and request a determination of eligibility no later than 21 days before bid submission. Insurance requirements include Workers' Compensation and Employer's Liability coverage ($1,000,000 minimum), Commercial General Liability ($1,000,000 per occurrence, $2,000,000 aggregate), and Automobile Liability ($1,000,000 per occurrence/aggregate), all issued by companies admitted in the Commonwealth of Virginia with an A.M.
