About this opportunity
The West Virginia Department of Administration, General Services Division issued Request for Quotation GSD1600000092 on December 16, 2015, for snow removal services at Building 54 located at 400 Adams Street in Fairmont, Marion County. Bids were due by 2:00 PM on December 21, 2015, submitted via fax to (304) 558-1475 or email to Carolyn.S.Hager@wv.gov. The procurement sought snow removal and de-icing services across three service categories based on snowfall accumulation levels: de-icing treatment for accumulation under one inch, de-icing and plowing for accumulation between one and six inches, and de-icing and plowing for accumulation exceeding six inches. Each service line item was quoted on an as-needed basis with an estimated quantity of 10 events per category. The contractor was required to supply all tools, equipment, personal safety equipment, and materials, including de-icing product effective down to negative 25 degrees Fahrenheit, with Material Safety Data Sheets to be submitted prior to beginning work. Award was to be made to the lowest total lump sum bid incorporating all costs including travel time and expenses. Prospective bidders could view the property by contacting Jeff Swisher at 304-543-2046, and inquiries regarding the solicitation were directed to Carolyn Hager at (304) 957-7137 or carolyn.s.hager@wv.gov.
The solicitation did not specify set-aside designations or restrictions on vendor eligibility based on business classification. All prospective vendors were required to submit a signed and notarized Purchasing Affidavit prior to contract award, certifying under penalty of law under W. Va. Code 5A-3-10a that neither the vendor nor any related party owed qualifying debt in excess of one thousand dollars in the aggregate or was in employer default, with limited exceptions for contested taxes and compliant payment plans. The successful bidder would be required to sign a WV-48 Agreement form incorporating the bid and RFQ as attachments. The resulting contract would be effective for one year from the effective date with two subsequent renewal years available if agreed to by both parties in writing; any price adjustments would only be allowed at renewal upon mutual written consent. The solicitation documents did not identify an incumbent contractor or specify an award value or budget range. Monthly invoices submitted in arrears were to be mailed to the General Services Division at 1900 Kanawha Boulevard East, Building 1, Room MB-60, Charleston, West Virginia 25305, or emailed to GSDinvoices@wv.gov.
