Eligible applicants are businesses, nonprofit organizations, and self-employed individuals who meet specified experience, education, and training requirements. Scope covers job placement, job coaching, retention services, community-based work assessments, targeted vocational assessments, job search assistance, and coaching in support of pre-employment transition services. Contractors must carry commercial general liability, automobile liability, and workers' compensation insurance as specified in Exhibit C. The contract prohibits self-dealing and placement within Oregon Forward facilities.
The resulting contract is outcomes-based, with a maximum not-to-exceed compensation amount. Payment components include referral acceptance fees, Job Placement Plan approval fees, monthly job placement report fees, placement fees, retention fees, and hourly rates for job coaching. Federal funds under Assistance Listing 84.125 support the contract, and the contract term and any renewal options appear in each resulting contract.
Addendum 1 extended the question deadline to December 31, 2031 and replaced Attachment H (Sample Contract) in its entirety. Written protests of the addendum are due 5:00 p.m. PDT on October 30, 2026.
