About this file

Summary

This is an Addendum 1 to a Request for Application (RFA) issued by the Oregon Department of Human Services (ODHS), Office of Vocational Rehabilitation (VR), soliciting applications from qualified providers to deliver Job Placement Services (JPS) to VR participants with disabilities. The RFA seeks applications from businesses, non-profit organizations, and self-employed individuals meeting specified experience, education, and training requirements. The amendment makes minor corrections to key dates: the final deadline for questions or requests for clarification has been extended to December 31, 2031 at 3:00 PM Pacific Time, and the final date for current JPS contractors to submit applications to avoid a service gap has been set to January 31, 2027 at 3:00 PM PST. The amendment also replaces Attachment H (Sample Contract) in its entirety with an updated version. Prospective applicants may submit written protests to the amendment by 5:00 PM PDT on Friday, October 30, 2026. The resulting contract will provide Competitive Integrated Employment Job Placement Services including job placement, job coaching, retention services, community-based work assessments, targeted vocational assessments, job search assistance, and coaching in support of pre-employment transition services for eligible VR participants.

The Sample Contract detailed in the RFA establishes an outcomes-based relationship with a maximum not-to-exceed compensation amount and payment structure based on successful completion of specific deliverables: referral acceptance fees, Job Placement Plan approval fees, monthly job placement report fees, placement fees, retention fees, and hourly rates for job coaching services. Contractors must maintain commercial general liability insurance, automobile liability insurance, and workers' compensation insurance as specified in Exhibit C. All contractor staff, including approved subcontractors, must complete mandatory trainings (Job Placement Services Contract Training courses and twelve Core Competency modules), pass background checks, and maintain six continuing education credit hours annually. The contract prohibits self-dealing and placement within Oregon Forward facilities, requires use of VR standardized forms, and mandates adherence to strict timelines for plan development, monthly reporting, and service delivery. Federal funds identified as Assistance Listings 84.125 will support this contract, with the contract term and any renewal options to be specified in individual resulting contracts.
