About this opportunity

The New Hampshire Department of Health and Human Services, Division of Long Term Supports and Services, is soliciting responses to Request for Applications RFA-2027-DLTSS-03-MFPTC to procure transitional case management services for the Money Follows the Person (MFP) program. The procurement seeks to award up to eight contracts to qualified vendors to serve one or more of ten designated New Hampshire counties: Belknap, Carroll, Cheshire, Coos, Grafton, Hillsborough, Merrimack, Rockingham, Strafford, and Sullivan. The services will provide comprehensive transitional case management to individuals over age 60 or between ages 18-60 with chronic illnesses or disabilities who meet Medicaid long-term supports and services eligibility criteria and are transitioning from institutional settings to community-based residences. Pre-transition services include comprehensive assessments, care plan development, risk identification, housing support, and connection to community resources, while post-transition support encompasses monthly monitoring, service coordination, crisis management, and benefit assistance over a 365-day program enrollment period. Contractors must maintain business hours operations from 8 AM to 5 PM, Monday through Friday, and provide a 24/7 toll-free telephone line, with service delivery requirements including face-to-face meetings twice monthly during pre-transition phases and minimum monthly face-to-face contacts during the first three months post-transition. Supplemental Medicaid billable services include funding for housing-related expenses such as transportation, application fees, home accessibility modifications, security deposits, utility deposits, household establishment items (with specific per-person cost caps), and initial pantry and kitchen stocking. The deadline for Letters of Intent and vendor questions is April 23, 2026, with vendor question responses due at 12:00 PM Eastern Time, and the solicitation response due date is May 15, 2026 at 12:00 PM Eastern Time. Responses will be evaluated on a 1,000-point scale based on three criteria: Ability to Perform Scope of Services (400 points), Experience and Knowledge (300 points), and Capacity (300 points), with a minimum qualifying score of 575 points required to advance to further consideration.

Prospective vendors must hold a current license under He-E 805 requirements and be enrolled as Medicaid Choices for Independence case management providers; no specific set-aside designations for disadvantaged enterprises are identified in the procurement documents. All contractor personnel must complete criminal background checks and state registry searches prior to service delivery, and contractors must comply with federal MFP requirements, HIPAA Privacy Rule standards, New Hampshire Information Security Requirements, and Title VI Civil Rights Act provisions regarding Language Assistance Services for Limited English Proficiency persons. Vendors must maintain commercial general liability insurance of not less than $1,000,000 per occurrence and $2,000,000 aggregate, comply with Workers' Compensation requirements, and execute a Health Insurance Portability and Accountability Act Business Associate Agreement. The total anticipated funding across all awarded contracts is $277,095, comprised of $168,210 in State Fiscal Year 2027 and $108,885 in State Fiscal Year 2028, with compensation at $452.24 per individual for both pre-transition and post-transition services during the three-month period, based on actual unduplicated individuals served. The Department anticipates serving approximately twelve individuals in calendar year 2026 and twenty-three individuals in 2027, with contract effective dates contingent upon Governor and Executive Council approval and a contract end date of December 31, 2027; the Department may extend contracted services for up to four additional years. Beginning in month four of service, vendors must bill Medicaid for current monthly Contextual Family Intervention Case Management rates and supplemental services in accordance with specified caps, with all pre-transition expenses requiring Department pre-authorization.
