Background

1.4.1. New Hampshire Department of Health and Human Services, Division of Long Term Supports and Services, Bureau of Adult and Aging Services

The New Hampshire Department of Health and Human Services’ (DHHS) mission is to join communities and families in providing opportunities for citizens to achieve health and independence.

The Department is responsible for promoting the health, safety and well-being of the residents of New Hampshire. To achieve that goal, the Department provides services for individuals, children, families, and seniors in mental health, developmental disabilities, substance misuse, and public health. This is accomplished through partnerships with families, community groups, private providers, other state and local government entities, and New Hampshire citizens.

The Bureau of Adult and Aging Services (BAAS) provides a variety of social and long-term supports to adults aged sixty (60) and older and to adults between the ages of eighteen (18) and sixty (60) who have a chronic illness or disability. Supports and services can be accessed through contracted Vendors, Aging and Disability Resource Centers, and New Hampshire DHHS/BAAS District Offices. Services and supports are intended to assist people to live as independently as possible.

1.4.2. Objective

MFP is a demonstration program with goals that include rebalancing the utilization of home and community-based services (HCBS) rather than institutional long-term supports and services; eliminating barriers that prevent or restrict the flexible use of Medicaid funds to support Medicaid-eligible individuals to receive supports and services

RFA-2027-DLTSS-03-MFPTC

Page 5 of 17 v.2.1 in the settings of their choice; increasing the ability of state Medicaid programs to ensure continued provision of HCBS to eligible individuals and ensuring procedures are in place to provide quality assurance activities for eligible individuals receiving Medicaid HCBS and to provide for continuous quality improvement of HCBS.

The Department intends to provide transitional case management services to support individuals participating in the MFP program to transition from receiving long term supports and services (LTSS) in an institutional setting to receiving LTSS services in community-based residences.

All supports and services in this RFA aim to assist eligible individuals to increase their independence and receive needed LTSS in the settings of their choice while ensuring safety and dignity.

1.4.3. Covered Population

Individuals who are over the age of sixty (60) or between the ages of eighteen (18) and sixty (60) with a chronic illness or disability, who meet Medicaid LTSS medical and financial eligibility criteria, are residing in a MFP qualified institution for at least sixty (60) continuous days, are assessed to need assistance to transition to a community-based setting, and determined to be eligible for participation in the MFP program.

The Department anticipates that approximately twelve (12) individuals will receive community-based transition services in calendar year 2026 and twenty-three (23) individuals in calendar year 2027.
