About this opportunity
The City of Green Bay Metro Fire Department (GBMFD) has issued Request for Proposal (RFP) #2023-38 seeking qualified medical screening providers to conduct comprehensive occupational medical evaluations for approximately 212 existing firefighters and 10 new firefighters annually. Required services include comprehensive physicals encompassing medical history, physical examination, blood tests, urinalysis, vision and hearing tests, spirometry, chest x-ray, electrocardiogram, cardiac stress testing, and skin cancer screening. For new hire physicals, additional requirements include a 5-panel non-DOT drug screen and post-offer functional assessment. Vendors must be licensed physicians with completed residency training or board certification. The RFP was issued on August 24, 2023, with written questions due by September 11, 2023, and sealed proposals required by 2:00 PM Central Time on September 21, 2023. Evaluation criteria will assess vendors on a 100-point scale: Meets Specifications (30 points), Previous Experience with NFPA 1582 Compliant Physicals (25 points), On-Site Exams capability (15 points), Price (20 points), and References (10 points).
The contract will have an initial three-year term from March 1, 2024, to February 28, 2027, with options to renew for two additional one-year periods. Vendors must provide proof of professional liability insurance ($1,000,000 per claim/$3,000,000 aggregate), general liability coverage ($1,000,000), business automobile coverage ($1,000,000), and workers' compensation coverage. Based on the evaluation summary, two vendors were considered: Bellin Health and Site Med, with Bellin Health scoring highest at 91.67 points compared to Site Med's 81.67 points. Bellin Health's pricing was $410.00 per evaluation under Option A and $701.00 per evaluation under Option B for as-needed services. The City reserves the right to reject all proposals, negotiate terms, or award to multiple vendors. The recommendation for contract award is scheduled for approval at the Finance Committee meeting on December 12, 2023, with a projected contract start date of March 1, 2024, pending City Council approval. Either party may cancel the contract with 90 days' written notice, and pricing shall remain firm for the initial three-year term with potential negotiated increases for renewal periods.
