The selected provider must hold a Doctor of Physical Therapy (DPT) degree and current state licensure as a Physical Therapist in Utah — the mandatory credential filter.

Proposers must provide minimum three professional references from public safety agencies or comparable organizations and hold Commercial General Liability insurance of at least $2 million per occurrence and Workers' Compensation with statutory limits.

The contract term is one year with four additional one-year renewal options. Project start is anticipated November 1, 2026, with contract negotiations scheduled October 29, 2026, and evaluation completed by October 20, 2026. Questions are due October 6, 2026 at 2:00 p.m. MT. Proposals must include separate technical and cost sections submitted via the Utah Procurement Place (U3P) website and are evaluated on experience and organizational qualifications (20%), assigned team qualifications and experience (20%), proposed approach and methodology (30%), relevant public safety experience and references (20%), and cost proposal (10%).
