About this opportunity
The West Virginia Department of Health and Human Resources, on behalf of Hopemont Hospital located in Terra Alta, West Virginia, is soliciting quotations for an open-end master agreement to supply dishwasher dispensing equipment and supplies. The procurement seeks bids for commercial dishwashing chemicals including Ecolab Brand Solid Power Plus (75 cases of 4-9LB containers), Jet Dry (60 cases of 4-1 gallon containers), Oasis 146 Multi-Quat Sanitizer (10 cases of 2.5-gallon containers), Pantastic Detergent (10 cases of 4-1 gallon containers), Universal Day Dots (40 rolls of 500 per roll), and automatic dispensing equipment to be provided, installed, and maintained at no charge for use with a Hobart #CL44E dishwasher, along with commercial oven degreaser. All shipments must be delivered within two business days of order placement to Hopemont Hospital's Central Receiving at 150 Hopemont Drive, delivery is F.O.B. destination with all shipping costs included in the quoted price, and vendors must accept orders through multiple methods including wvOASIS, mail, facsimile, and email. The contract will be awarded to the vendor providing the lowest overall grand total cost for all contract items, and vendors must be registered with the West Virginia Purchasing Division prior to award and maintain sufficient inventory to fulfill obligations with quarterly reports and annual summaries of purchases required. Questions must be submitted in writing by Friday, August 7, 2015 at 4:00 p.m., and bids are due by Friday, August 14, 2015 at noon; the contract term is one year from the award date with options for three successive one-year renewal periods not to exceed 36 months total.
Vendors may apply for preferential consideration through West Virginia's Vendor Preference Certificate program, which provides preferences ranging from 2.5 to 5 percent based on residency status, in-state employment practices, veteran status, or certification as small, women-, and minority-owned businesses, with preference applied only to the cost bid submitted by vendors. All prospective vendors must complete and have notarized a Purchasing Affidavit affirming under penalty of law that neither the vendor nor any related parties owe debts exceeding one thousand dollars in aggregate to the state or its political subdivisions and are not in employer default status, with limited exceptions for contested taxes or approved repayment agreements. Commercial general liability insurance in the amount of $250,000 or more is required, and vendors must maintain workers' compensation insurance in accordance with applicable laws. Pricing must be firm for the life of the contract unless otherwise authorized by the state in writing, and all bids must be signed and acknowledge receipt of any addenda issued with the solicitation to avoid disqualification. No incumbent vendors, set-asides for specific business categories, or specific award values or budget ranges are identified in the solicitation documents.
