Place of performance includes four service locations: Veterans Health Care System of the Ozarks in Fayetteville, Arkansas (daily pickups); Fort Smith Community-Based Outpatient Clinic (weekly); Springfield Community-Based Outpatient Clinic in Missouri (biweekly); and Joplin Community-Based Outpatient Clinic in Missouri (biweekly). The contract guarantees a minimum value of $2,500 and establishes a maximum capacity of $4,000,000.
The contractor must furnish all labor, tools, materials, equipment, contractor-owned reusable containers, transportation, treatment via autoclave or incineration, and disposal services in compliance with OSHA, EPA, DOT, state, and local regulatory requirements. The contractor must maintain 24/7 emergency response capability with qualified personnel onsite within two hours of notification, provide next-business-day container returns at Fayetteville, submit monthly manifests and invoices reconciled to pickup documentation, and possess all required state transporter permits and vehicle approvals prior to performance. All offerors must submit quotations covering all contract line items and service locations; partial quotations will not be accepted. Technical questions are due August 26 at 1:00 PM CDT. Evaluation criteria include technical capability and quality control, relevant experience with similar projects, compliant subcontracting management plan, and price.
