Amendment FA4855-17-R-0008-0004
The purpose of amendment 0004 to FA4855-17-R-0008 is to:
a. Revise Attachment 13 - Questions and Answers. The revised document incorporates industry questions on the RFP that were answered by the Government since Amendment 0003.
b. Revise Attachment 1 - Contractor Team Organization for the purpose of making administrative corrections to Performance Work Statement (PWS) paragraph references.
c. Revise Attachment 9 - Performance Work Statement for the purpose of correcting and clarifying the Government's requirement.
d. Revise Attachment 11 - Consolidated Government Furnished Property List for the purpose of correcting and clarifying the Government's Government Furnished Property.

The purpose of amendment 0003 to FA4855-17-R-0008 is to:
a. Revise Attachment 13 - "Questions and Answers" document to the RFP. The revised document incorporates industry questions on the RFP that were answered by the Government since the RFP release date of 22 May 2017.
b. Revise addendum to 52.212-2 for the purpose of clarifying and correcting the Government's past performance evaluation methods.
c. Revise Attachment 3 - Price Proposal Workbook for the purpose of clarifying and correcting the Government's price evaluation methods.
d. The proposal due date has been extended from 1400 MST 21 June 2017 to 1400 MST 26 June 2017.

The purpose of Amendment 0002 to FA4855-17-R-0008 is to:
a. Revise Attachment 5- "Past Performance Questionnaire." The original document paragraphs 3 and C. "RETURN COMPLETED SURVEY FORM TO" contain incorrect email addresses which have been corrected from "shane.tittinger.1@cannon.af.mil" and "brittney.pastore.2@cannon.af.mil" to "shane.tittinger.1@us.af.mil" and "brittney.pastore.2@us.af.mil."
b. Revise Attachment 2- "Financial Responsibility Questionnaire." The original document under the subheading "Financial Institution" referring to Lt Shane Tittinger contains an incorrect email address which has been corrected from "shane.tittinger@us.af.mil" to
"shane.tittinger.1@us.af.mil."
c. The remainder of Attachment 5- Past Performance Questionnaire remains unchanged. The remainder of Attachment 2- Financial Responsibility Questionnaire remains unchanged. There are no changes to the remainder of the solicitation.

2 JUN 2017 | SITE VISIT ATTENDEES:
Please arrive at the Cannon Air Force Base Main Entrance Visitor Center no later than 7:30 am. Our team will meet you there and process visitor passes. We will then take a bus from the Visitor Center to Melrose Air Force Range for the site visit. The bus will depart for the MAFR Site Visit no later than 8:00am.

The purpose of amendment 01 to FA4855-17-R-0008 is as follows:
a. To include an Attachment 13 - "Questions and Answers" document to the RFP. The document incorporates industry questions on the RFP that were answered by the Government between the RFP release date of 22 May 2017 and 31 May 2017; and
b. To incorporate revised addendums to 52.212-1 and 52.212-2 for the purpose of clarifying and correcting the Government's past performance evaluation methods.

PURPOSE OF NOTICE: This is a request for Proposal (RFP) for FA4855-17-R-0008 issued by the 27th Special Operations Contracting Squadron at 110 E. Alison Ave, Cannon AFB, NM, 88103 for Melrose Air Force Range (MAFR) Operations, Maintenance, and Support (OM&S) Services. The Government anticipates one contract to be awarded as a competitive 8(a) small business set-aside IAW FAR 19.502-2(b). The Government anticipates utilizing North American Industry Classification System (NAICS) code 561210, Facility Support Services. The proposed contract action is for commercial items prepared in accordance with Subpart 12 and is for services which the Government intends to solicit using Tradeoff source selection procedures.
PERIOD OF PERFORMANCE: 1 December 2017 through 30 November 2018 with three (3) additional one-year option periods.
BRIEF DESCRIPTION OF SERVICES: Contractor shall provide a fully functional primary training range to meet war-fighter training and readiness requirements. The contractor shall provide personnel, training, and supervision to perform three regimes of support encompassing ground, electronic/technologic, and administrative services. These regimes include all operations, maintenance, support (OM&S), air and ground scheduling, paramedic services, and wildland fire management for MAFR. In addition, the contractor is responsible for supporting operation and maintenance of all range equipment including vehicles and heavy equipment used to support the operations of MAFR.
ADDITIONAL NOTES: The Government will conduct any source selection in accordance with Air Force Federal Acquisition Regulation Supplement (AFFARS) 5315 and the attached RFP. A pre-proposal site visit will be held on June 2, 2017 at 0900 MST IAW the RFP.
Additional details for pre-proposal site visit:
Offerors intending to attend the site visit on 2 June 2017 must notify the solicitation POCs with names of personnel planning to attend NLT 31 May 2017.
Please keep in mind:
Pictures will not be permitted on MAFR. Please do not bring cameras or cell phones.
Wear long pants and sturdy shoes.
Bring bottled water. It's projected to be hot, dry, and windy. MAFR doesnot have a potable water source.

PURPOSE OF NOTICE: The 27th Special Operations Contracting Squadron at 110 E. Alison Ave, Cannon AFB, NM, 88103 intends to solicit for Melrose Air Force Range Support Services. The Government anticipates one contract to be awarded as a competitive 8(a) small business set-aside IAW FAR 19.502-2(b). The Government anticipates utilizing North American Industry Classification System (NAICS) code 561210, Facility Support Services. The formal request for proposal will be under Solicitation Number FA4855-17-R-0008 and is anticipated on or about 26 May 2017.
PERIOD OF PERFORMANCE: 1 December 2017 through 30 November 2018 with three (3) additional one-year option periods.
BRIEF DESCRIPTION OF SERVICES: Contractor shall provide a fully functional primary training range to meet war-fighter training and readiness requirements. CLEARANCE REQUIREMENTS: Contract security requirements and contractor access to classified information shall be in accordance with the National Industrial Security Program Operating Manual (NISPOM) and as specified in the DD Form 254 - Contract Security Classification Specification.
PLACE OF PERFORMANCE: Cannon Air Force Base (CAFB), Melrose Air Force Range (MAFR) and local surrounding areas.
ADDITIONAL NOTES: In order to be awarded a contract by this office an offeror must be registered in the System for Award Management (SAM). Registration is free and can be completed online at www.sam.gov. Note that you must have a Data Universal Numbering System (DUNS) Number before you can register. Failure to be SAM registered at time of award may delay the award and depending on the need of the government may be because to award to the next qualified best value offeror already registered. However, do not delay submitting your proposal pending confirmation of registration. Include your DUNS number and CAGE code along with your proposal as proof of active registration.
