About this opportunity
The Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 10 seeks Lock Out–Tag Out (LOTO) performance-based services at Chillicothe VAMC, with quotations due September 17, 2026 at 3:00 PM ET. The procurement is set-aside 100% for Service-Disabled Veteran-Owned Small Businesses and is issued as a Request for Quotation under the streamlined commercial procedures of FAR subpart 12.202.
Place of performance is Chillicothe, Ohio (17273 State Route 104, Chillicothe, OH 45601). Work is performed Monday–Friday, 7:30 AM to 4:00 PM, excluding federal holidays, with check-in at Engineering Department (Building 21, NE side).
The contractor must develop, implement, and annually review graphical LOTO procedures for equipment at the facility in compliance with OSHA 29 CFR 1910.147, OSHA 29 CFR 1910.301–399 Subpart S, and ANSI Z244.1 standards. The base year (October 1, 2026 through September 30, 2027) covers 137 initial equipment assets; each of four one-year option years adds a minimum of 90 additional assets for a potential total of 454 assets over the five-year contract period. Deliverables include minimum 454 equipment-specific graphical procedures developed through direct field verification with VA electricians (7–8 hours per day availability), durable 14.5" x 9" laminated placards with grommet attachment in standard format, two indexed and tabbed documentation binders with digital and hard copy files, asset labels affixed to all covered equipment, and annual in-person training for approximately 75 Engineering and Facilities Management employees. The contractor must establish a Quality Control Program compliant with OSHA standards with 90% compliance for procedure development and placard attachment; deficiencies identified during annual audits must be corrected within 10 business days.
Contractors must demonstrate documented experience with electrical safety programs and implementation of OSHA/ANSI LOTO programs in healthcare or similar environments. Service Contract Act wage determination 2015-4771, Revision 34 (Ross County, effective August 17, 2026) applies. The contract will be Firm-Fixed-Price.
SDVOSB contractors are limited to 50% subcontractor payments to non-SDVOSB/VOSB firms and must complete and return VAAR clause 852.219-75 certification with quotations. Evaluation prioritizes Technical Capabilities and Past Performance over Price; the Government may select a higher-priced quotation if additional technical merit or past performance value warrants the cost. Requests for Information were due September 8, 2026; Amendment 0001 extended the quotation due date from September 10 to September 17, 2026, and Amendment 0002 (September 14, 2026) posted VA responses to RFIs and revised Performance Work Statement, schedule of line items, and wage determination.
