The Canandaigua VA Medical Center, located in New York State, has a need for currently active Low Demand Safe Haven (LDSH) facility services as part of its community-based Health Care for Homeless Veterans (HCHV) program. The facility needs to be currently operating in order to be initially inspected by the VA to ensure operations comply with Federal law and regulations and HCHV inspection standards in accordance with VHA Directive 1162.04. The goal of this HCHV program is to target the population of hard-to-reach chronically homeless veterans with mental illness and/or substance use disorder (SUD), when traditional residential treatment programs do not meet a Veteran's needs. The low demand or nonintrusive environment is designed to establish trust and motivate chronically homeless veterans to seek needed treatment services and transitional and/or permanent housing options. The contractor facility must be located within a 15-mile radius of the Canandaigua VAMC.
