About this file
This is a Construction Bid Submission Review Form created by the State of West Virginia Purchasing Division on June 18, 2012, designed to serve as an informational guide for contractors submitting bids on state construction projects. The form outlines common bidding problems and requirements that contractors must satisfy to avoid bid disqualification. The document explicitly states that it is not a comprehensive list of requirements for any specific project and does not cover all potential reasons for bid rejection. Rather, it serves as a reference tool highlighting the most frequently encountered issues in the state's construction bidding process. All bidders are required to thoroughly review the complete Request for Quotation documents, including all addenda, instructions, and supporting materials, to identify the actual bidding requirements applicable to their specific project.
The form identifies ten errors that shall result in immediate bid disqualification, including failure to attend mandatory pre-bid meetings, failure to sign the bid, failure to provide a West Virginia contractor's license number, failure to submit a signed drug-free workplace affidavit, failure to provide a valid bid bond, failure to meet any mandatory RFQ requirement, failure to acknowledge receipt of addenda when required, failure to submit the bid by the deadline, federal debarment, and state debarment or suspension. Additionally, the form lists five errors that may result in bid disqualification before contract award, including uncontested state debt exceeding $1,000, workers' compensation or unemployment compensation delinquency, failure to register as a vendor with the state, failure to obtain required bonds and insurance, and failure to provide the sub-contractor listing within one business day of bid opening.
