MEMORANDUM FOR 4 CES/CEIEC

ATTENTION: DENNIS EVANS

FROM: 4 CES/CEOE

SUBJECT: Facility Inspection/Assessment Letter

1. The following information is provided per your request for asbestos and or lead paint inspection or sampling in facility 4534, 4MXG/EMS SHOP AIRCRAFT, GENERAL PURPOSE for WTO 15097399, Office Area Carpet Replacement.

2. A visual inspection and data base inquiry was performed, and samples were gathered on July 23, 2024. Prior bulk samples are on file for this facility and interior building materials. Asbestos was found in the sampled suspect material. The floor tile mastic under the VCT tile under the carpet to be demolished was found to be 8% Chrysotile.

3. The work area is NOT clear of asbestos containing building materials. If any of the VCT tiles are disturbed during carpet demolition, asbestos abatement by the shops will be required. You may contact www.osha.gov, David McLamore @ 919-790-8096, North Carolina HHCB @ 919-707-5950 or the Abatement Shop for further asbestos or lead guidance.

4. This information is in reference to the materials and area identified above (see attached floor plan). It does not cover materials or areas not identified or accessible for inspection or sampling. Other materials may contain asbestos or lead and must not be disturbed until an inspection can be performed to determine the potential health risk.

This assessment is valid for seven years or invalid if the original scope of work changes.
